LPN Scheduling Coordinator
- Accepts routine intake for referrals from insurance plans, hospital discharge planners, etc. in the event clinical information needs to be exchanged RN to RN referral will be passed to RN coordinator
- Makes calls to field staff to staff referral
- Verifies insurance information on referrals reports problems with insurance to RN coordinator
- Maintains logbook for referrals
- Follows up on incorrect addresses, phone numbers once identified
- Accepts authorization numbers for referrals and records on the referral
- Makes calls to insurance plans to notify of refused referrals, turns backed referrals, need for additional referrals as needed or other issues
- Accepts and files all field paperwork
- Screens files for deficiency and any other inappropriate issues. Follows up on the deficiency corrections. Creates appropriate logs to monitor performance improvement
- Maintains schedules and geographic coverage for staff
- Assist the billing department in the follow up of patient referrals not accounted for at the end of the billing cycles
- Makes HHC-3000 data entries
- Ensures information is communicated to field staff, office staff, and customers in a timely manner. Participate in on call duties.
- Performs specific tasks as assigned. Acts as a liaison between field staff, hospital discharge planners, insurance company reps and office staff (20%) Provides administrative support to Clinical Management staff (10%) Responsible for intake, staffing and follow up of routine referrals, client’s file management and maintains (70%). Performs PPD'S and hosts nursing orientations and performs skills checks for HHA.
