LVN - Continuing Care, Long Term Care
Responsibilities: Cover Bed placement which includes review of patient charts for discharge from the hospital and transition to the Skilled Nursing facility (SNF). Creating inquiry packets with relevant clinical information for placement to a SNF. Coordination with the SNF for any DME, medication, transportation, dialysis coordination needs, or any other patient need that may arise. Occasional duties may include creation/follow-up for Letters of Agreement (LOA with non contracted SNFs), chart audits, clinical data collection/trending, custodial patient tracking and connecting with RN for any follow up needs. Review of OURS patients needing SNF placement.
- Float department (if needed): The Continuing Care Department contains sub departments including Long Term Care (SNF), COMC (aka Dementia program, telephonic follow up with patients in the community), Living Well (High utilizer program, patients in the community), SNP (Special Needs Population, aka frail/elderly, KP Medicare/Medi-Cal members)
Experience
- 1 year of experience in LVN/LPN
- SNF experience Preferred
License and certification
- CNA License
- BLS

Pay: Up to $40.00 per hour
