We're looking for someone to join our team as a LVN, Care Management Coordinator (Home Health) who:
Coordinates incoming office referrals and verifies insurance coverage. Coordinates incoming calls from patients, family, physicians, and disciplines. Ensures that all personnel are informed of patient's hospitalization and follow-up with patients admitted to other hospitals. Coordinates small program(s) with limited budget/impact. Applies substantial knowledge and experience to perform a wide range of advanced activities and/or determines how to use resources to meet schedules and goals; serves as working supervisor for team or work group.
Essential Functions:
- Handles and coordinates incoming calls related to patients, physicians, referrals and agency services regarding physician orders, patient questions and patient referrals.
- Facilitates telephone communications between, and demonstrates effective communication skills, with nursing, therapy, aide, social services, physicians, etc., regarding changes in patient/staff schedule, test results, etc.
- Assists with staffing/scheduling activities, soliciting input from managers.
- Provides follow-up for hospitalized patients, ensuring subsequent agency referral.
- Performs other job-related duties as assigned.
You'll be successful with the following qualifications:
Education:
- High School Education/GED or equivalent: Preferred
- Associate's/Technical Degree or equivalent combination of education/related experience: Preferred
Work Experience:
- Experience in a healthcare setting: Preferred
Required Licenses/Certifications:
- Licensed Vocational Nurse (LVN) licensure in the state of practice: Required

The estimated base pay for this position is $21.14 to $31.68. Additional individual compensation may be available for this role through years of experience, differentials, extra shift incentives, bonuses, etc. Base pay is only a portion of the total rewards package and a comprehensive benefits program is available for qualifying positions.
