SCHEDULE FOR THIS POSITION: Mon-Fri 8a-4p (40 Hours Per Week)


PRIMARY PURPOSE:
Coordinate efforts to enhance the quality of services in the Community Living Division (Residential, ISS, Shared Living & AFC). Facilitate compliance, human rights, and training efforts for staff and persons served.

KEY ACTIVITIES / Essential Functions (Performance Measurements):


  • Facilitate programmatic compliance efforts within Residential I/DD and ABI Programs:
    • Applies to following entities:
      DDS, OEQ Licensing and Certification, ABI, U/MASS Waiver, HIPAA, and FHCRC policies.
    • Facilitate/Coordinate, document, and track regular (as determined by QA Director) audits of Residential Programs:
      • Financial (in conjunction with Rep Payees)
      • MAP (Conduct and/or coordinate with RNs)
      • Data collection (ISP, Behavior Interventions, & other efforts requiring ongoing data collection)
      • OQE Licensing & Certification
      • Safety (Home, Fire, & Related Documentation)
      • Assist Residential leadership during Licensing, Certification, MAP, DPH, and other program audits
      • Develop & Implement follow-up action plans related to internal and external program audits.
  • Facilitate Training for Residential Programs (Employees and Persons Served):
    • Deliver training as needed to employees:
      • Complete and Maintain Train the Trainer Certifications as needed
      • Program Specific Training:
        • Develop and monitor list of diagnoses, equipment, and interventions within residential programs requiring staff training- update quarterly and disseminate for implementation
        • Track and Report on Program Specific training
      • Deliver trainings as required:
        • First Aid / CPR
        • De-Escalation / Behavior Management
        • Human Rights / DPPC
        • Program/Individual Specific (if required)
        • New Hire Orientation
      • Assist QA Director to track and report to Res Leadership on employee training
    • Persons Served Training:
      • Track and Report on Mandatory Trainings in Residential Programs
        (Fire Safety, Human Rights, Grievance/Appeal, Anti-Harassment, others defined by QA Director)
      • Facilitate Self-Advocacy Forums
      • Create and Disseminate monthly Human Rights Curriculum to HR Officers across Residential (Written & Video Materials)
  • Assist QA Director with Information Management Activities
    • Monitor and Report on use of database for data collection
      • Daily Notes
      • ISP data
      • Behavior Interventions
      • Medical Interventions requiring data collection
      • Etc. as identified by QA Director
    • Ensure HCSIS, DPPC, MAP reporting is completed to coincide with data collection
    • Assist QA Director with Database training, Implementation of new modules, & password management
    • Attend Monthly- Introducing Safety concerns and identifying solutions
  • Facilitate efforts to identify and enhance Stakeholder Satisfaction across Residential
    • Facilitate annual satisfaction surveys to all stakeholder groups across Residential
      Distribute annually, Tabulation, Report and Disseminate
    • Obtain and track quarterly feedback on employees by persons served
    • Assist QA Director to develop and disseminate annual Residential-specific QA report with highlights and recommendations for continued improvements.
  • Facilitate monthly QA Forums with Residential VP, AVP, Senior Program Directors and Program Directors
    • Provide Updates
      Training, Database/Data Collection, Safety Committee Findings
    • Provide monthly summary of Program audits, along with trends
    • Monthly summary of Fire Drill Documentation
    • Implementation of new initiatives across Residential
    • Introduction of new policies / practices
    • Distribution of monthly Human Rights Curriculum
    • Distribution of QA Annual Residential Report


OTHER DUTIES AS ASSIGNED, INCLUDING BUT NOT LIMITED TO.

  • Assist in the coordination of Human Rights meetings, minutes and communication
  • Attend staff meetings as needed for trainings
  • Participate in self-advocacy efforts within departments
  • Attend various trainings as needed
  • Attend quarterly Safety Committee meetings
  • Participate in PBS, Crisis Response and other agency committees
  • Attend clinical meetings, staff meetings and residential meetings
  • Other Duties as assigned by QA Director


REQUIREMENTS:

  • Associates Degree in Human Services or related field required, Bachelors Degree preferred
  • Four years of experience in Human Services (I/DD, TBI/ABI, or Mental Health)
  • At least two years in a supervisory or administrative capacity
  • Excellent organizational, communication and leadership skills
  • Experience in Employee Training (Human Services preferred)
  • Proficiency in Excel, Power point, and case management software (as it relates to development of training materials and tracking of trainings)
  • Ability to work a flexible schedule (occasional evenings and weekends, capacity to work over 40 hrs weekly during peak times, and ability to accommodate varied staff/individual schedules)
  • Minimum of 1 year of driving experience and a valid drivers license
  • Regular and predictable attendance
