HOUSING CASE MANAGER JOB DESCRIPTION

Title: Housing Case Manager Department: Crisis Services

Type: part-time, hourly Reports To: Client Services Director /Shelter Manager

Job Summary: Responsible for implementation of individual case management and crisis intervention for the entire family. Will serve as the Case Manager from the time the family enters services (shelter or community) and up to 1 year after leaving shelter. Will have a focus in housing-based case management to help clients achieve and maintain permanent housing and sustainability.
Responsibilities: Completes Individual Client Assessment Packets (ICAP) with every adult that enters shelter, the VI-SPDAT, as well as the Children’s Intake and Individual Service Plan (ISP), if applicable, within 72 hours of entering shelter.

  • Completes housing assessment within the first full week of client entering shelter.
  • Utilizes coordinated assessment tools for local Continuum of Care including but not limited to SPDAT, central assessment, barriers assessment, etc. Attend family and single meetings. Present clients at both meetings
  • Performs direct case management with entire family/singles.
  • Reviews, prioritizes and takes appropriate action in advocating for clients as needed.
  • Creates and maintains collaborative partnerships with other service providers for the benefit of clients housing needs. Engages in collaborative efforts region wide for Domestic Violence (DV) prevention.
  • Uses agency workplan to guide program objectives and activities.
  • Maintains accurate and timely reporting of grant documentation, compliance, daily, weekly, monthly, quarterly and annual reports.
  • Coordinates and provides transportation for adults and children when needed.
  • Co-facilitates Support Groups for adults and/or children as needed.
  • Responsible for rotating childcare and/or managing volunteers for childcare during scheduled groups, court dates, employment searches, etc.
  • Maintains an understanding of domestic violence, mental health issues, child abuse/neglect, and Child Protective Service procedures.
  • Makes arrangements for school transportation and school enrollment.
  • Helps to identify and remove barriers to housing.
  • Help client review and choose affordable housing options.
  • Helps client build budget and life plan with the goal of achieving and maintaining housing.
  • Works with client to identify support resources to help stabilize and thrive in housing.
  • Assists clients with completing housing applications and housing search.
  • Continue providing Case Management up to 1 year after family exits shelter.
  • Develops home based case management goals, provides home visits, ongoing support as well as advocacy for families/singles after they leave shelter.
  • Actively participates in training seminars, staff meetings program meetings, speaking engagements and other agency functions.
  • Maintains a professional demeanor with supervisor, co-workers, clients and other organizationsrepresentatives. Communicates effectively and courteously. Brings appropriate issues to the Executive Director’s attention. Adheres to GS. policies and procedures, including the Standard of Conduct.
  • Works with community clients that are not in shelter but that still need case management services.
  • Performs all other duties as assigned

Requirements: Undergraduate degree in Human Services, Social Work, Counseling or related area preferred. Must possess strong proficiency in computer software programs. Case Management experience helpful.
Personal Qualifications:

  • Ability to work independently to accomplish program goals
  • Ability to collaborate with other service providers and maintain partnerships that benefit the clients and agency
  • Professional and sensitive attitude, which reflects the philosophy of the GS. Shelter.
  • Excellent driving record, valid driver’s license, certificate of insurance from an insurance carrier licensed in the state of Virginia
  • Knowledge of domestic violence issues
  • Ability to communicate orally and in writing
  • Ability to multitask and manage multiple needs of workload appropriately
  • Project Management experience helpful
  • Knowledge on Rapid Rehousing helpful
