Home Health Marketing Outreach Coordinator

Northern Homecare is looking for an outgoing, motivated, and professional individual to act as a liaison between our home care offices and other healthcare professionals within the southernmost area of Leelanau, Benzie, Manistee, Mason, parts of Oceana Counties, and more. We’re growing and need an amazing individual that is self-motivated, disciplined, doesn’t mind daily travel, and loves a challenge. If you fit this description, this position may be for you.

Requirements include:

• Applicants should be professional, outgoing, and comfortable with meeting new people every day
• Must have reliable transportation
• Must be willing to travel as far north as Traverse City, and as far south as Pentwater and possibly Muskegon.
• Experience with home care and/or marketing is preferred but not required, will train if needed
• Report in-person position

Responsibilities include:

• Identification of new patient referral sources
• Development and maintenance of existing and prospective professional relationships/accounts
• Identification of community outreach opportunities to promote our company as an active contributor and supporter of our area and its citizens
• Participation in care coordination to ensure the fluid transition of patients to and from other healthcare providers
• Regular communication with your office staff and clinicians to ensure continuity of care
