Job Summary

Join UT Southwestern as a Home Health Aide within our Home Health Department and help with our commitment to deliver compassionate care and all-around excellence. As a Home Health Aide, you will support the Home Health Department by providing excellent patient education for the patient and their families, working with patients under the direction of the RN’s and Physical Therapist.

Experience and Education

High school diploma or equivalent. Nurse’s Aide certification listed on the HHSC nurse aide registry (NAR). **Cardio-Pulmonary Resuscitation or Basic Life Support (BLS) certification, course accredited by the American Heart Association (AHA) or American Red Cross (ARC), valid TX Driver’s License and proof of driver’s insurance required. ** Minimum one (1) year of full-time experience in direct client care in an institutional setting (hospital or nursing facility). Minimum one (1) year of full-time experience within the last five (5) years in direct client care in an agency setting.

Job Duties

Performs duties as assigned on home health care plan prepared by registered nurse.
Obtains and documents patient vital signs, including temperature, weight, respiration, and pulse.
Reports changes in patient's mental or physical condition or in home situation to nursing supervisor.
Assists other members of nursing team and/or other disciplines.
Provides personal care assistance to patients.
Maintains patient records and charts to ensure accurate record of treatment.
Maintains confidentiality of patient data.
Assists in maintaining healthy, safe environment.
Attends mandatory inservice educational programs as required by federal and state regulations.
Performs other duties as assigned.
