Moorestown Visiting Nurse Association, a leading agency for comprehensive home care in southern New Jersey, is seeking a full-time RN Case Manager to join our team.

As an RN Case Manager, you will play an important role in our home healthcare services. This is a multifaceted position that will allow you to work alongside referring physicians to deliver direct care and provide the necessary support and education to patients and their families. Our Case Managers serve as liaisons for patients to evaluate and monitor their progress with the prescribed care plans, coordinating resources and ensuring proper billing. This role will allow you to work independently and utilize your home care experience and professional judgment to consistently meet best practice standards and established policies of the agency. This role will allow you to:

• Perform initial and ongoing compressive care assessments.
• Consult with patients and their families to identify care needs, develop care plans and identify short- and long-term care goals.
• Provide oversight of the IDT alongside an interdisciplinary team during patient conferences.
• Teach self-care to patients and families to ensure optimal results.
• Request home health aide services and write HHA care plans.
• Supervise the performance of home health aides and LPNs.
• Participate in home care on-call and rotate weekends and holiday call, as needed.

The successful candidates should be NJ-licensed registered nurses, preferably with a BSN, with at least one year of experience working in home health. Candidates will also need to have a valid driver’s license as this position requires traveling throughout Camden County.
