Responsibilities:

• Conducts clinical assessment and review in employee orientation.
• Provides scheduled in-service education for all field staff.
• Performs intakes for potential clients as needed.
• Performs patient and safety assessments on clients.
• Perform Start of Care/ Initial client home assessments and reassessments.
• Visits home care clients as required by accrediting board and documents appropriately.
• Ensures that all necessary documentation is completed accurately on each case.
• Monitors client clinical status and ensure that all files meet standards as per governing regulations.
• Collaborates with nursing team to teach certified home health aide classes.
• Coordinates with community agencies to provide continuity of care.
• Effectively communicates with the healthcare team, client, and family members.
• Communicates the client's health status to the client and family members.
• Participates in the on-call rotation ensuring that qualified personnel are available to respond to clinical issues or home visits.
• Ability to work independently and with minimal supervision, utilizes time effectively, maintaining a consistent level of productivity.

Experience and Education:

• Registered Nurse in good standing, holding a current NJ license by the NJ Board of Nursing.
• Proficiency in clinical assessments, documentation and compliance with nursing care, policies, and procedures.
• Full COVID vaccination required.
• A reliable vehicle and valid driver's license

Salary: $80,000-$90,000
