High Fidelity Wraparound Facilitator - Valencia County

Los Lunas, New Mexico. Must be a New Mexico resident.

Essential Functions:

Using high fidelity Wraparound values, principles and practices, the Wraparound Facilitator provides intensive, supportive interventions for identified families. Such interventions include running team meetings, coordinating care with other agencies, developing and utilizing informal and formal supports, identifying and making use of family strengths, etc. The ultimate goal of the NM Wraparound Program is to help the child/youth and family realize their vision and increase their autonomy from formal systems.

Specific Duties:

Specific Duties:

1. Engagement Skills:

  • Meet, greet and make child/youth and family feel welcome
  • Explain and clarify the Facilitator’s role, the Wraparound process and the role of services
  • Work closely with the family and other team members to help them understand the purpose and value system of the Wraparound approach and to encourage them to effectively participate in the process
  • Assess immediate risk for safety and coordinate resources for stabilizing that risk
  • Arrange for the collection of relevant information regarding the family’s story, current situation, strengths, needs and vision
  • Clarify and communicate the initial conditions that initiated referrals to the agency
  • Begin identifying and contacting team membership for planning team
  • Complete regulatory documents and outcome measures in a timely manner

2. Planning Skills:

  • Organize team members for a planning team
  • Engage team members in developing an overall plan of care that includes a vision statement, addresses initial conditions that brought the child/youth/family for help, builds on strengths, addresses needs, documents safety contingencies, details actions and specifies an evaluation process
  • Guide the team members towards open, strengths-based, family-friendly problem solving
  • Locate, engage and coordinate services and resources across the system of care
  • Coordinate and maintain necessary paperwork

3. Implementation Skills:

  • Facilitate communication among all team members in planning meetings regarding the follow through, timeliness and outcomes of planned intervention Coordinate and allocate resources (e.g. Flex funds) needed to implement the interventions outlined in the plan of care
  • Coordinate additional services (e.g., health, medication, psychological testing) and provide linkages/referral to these services as identified in plan of care
  • Fulfill care coordination expectations as requirement by each youth’s managed care organization (MCO), as indicated per contractual obligations between the MCO and All Faiths
  • Involve team members in decisions to make changes as needed
  • Maintain a healing and helpful relationship with child/youth and family
  • Guide team to implement the interventions of the plan and update as necessary based on an ongoing feedback and evaluation process
  • Address issues of team conflict and engagement in a timely manner

4. Transition Skills:

  • Empower team members to assess the appropriateness of a shift into the transition phase
  • Analyze progress toward the meeting of service goals
  • Review existing crisis response strategies
  • Empower family members to revisit and revise safety plan as needed
  • Prepare and distribute a written transition plan that communicates schedules for ending formal services and outlines follow-up and community resources available
  • Prepare all necessary reports for referring and participating agencies related to the close of service

  • Exercise tact and sensitivity in performance of job duties
  • Proactively work to ensure that all information is appropriately communicated
  • Treat all team members with respect, including arriving punctually to scheduled meetings; exhibiting flexibility in scheduling; following through on all assignments; communicating both honestly and positively; and avoiding gossip or participating in denigrating conversations

Position Requirements:

The NM Wraparound Program follows an 18-month training and coaching model. This intensive training and oversight prepares each participant to function fully and independently as a Wraparound Facilitator. As such, a Wraparound Facilitator and his/her employer must be able to make the following minimum commitment to the Program. The requirement are as follows:

1. Provide Wraparound Facilitation for a minimum of 3 families while participating in the program:

  • Prior to training identify the 1st family with whom you will work
  • You must begin working with 1st family within 6 weeks post-training

2. Attend all required trainings including:

  • Initial multi-day Facilitator training focused on Wraparound philosophy and skills
  • Initial Evaluation training within 2 months of initial Wraparound training
  • Attendance at all scheduled mandatory 1 or ½ day trainings
  • Participant’s direct supervisor to attend a Wraparound Overview training in first 3 months.

3. Participate in all monthly scheduled phone/in-person individual coaching sessions

4. Participate in all monthly scheduled phone/in-person group coaching sessions

5. Comply with all documentation requirements and evaluation/feedback processes

6. Assist in training future Wraparound Facilitator trainees, as requested

Knowledge, Skills and Abilities Preferred to Perform the Job:

1. Specialized knowledge and skills in the following areas:

  • child development, and individual and family functioning;
  • child abuse and domestic violence;
  • working with hard to reach or resistant individuals and families; and
  • collaboration with other disciplines and services.

2. Awareness and commitment to a best practice model that includes:

  • identifying, formulating and providing interventions in a manner that demonstrates respect for socio-cultural values, personal goals, life style choices and complex family interactions;
  • laws regarding mandated reporting of suspected abuse and neglect;
  • issues and responsibilities related to the “duty-to-warn.”

3. Competence in a variety of clinical areas including:

  • methods of engagement;
  • methods of crisis prevention and intervention;
  • identification of medical needs or problems;
  • the organization’s plans for dealing with medical or psychiatric emergencies;
  • knowledge of public assistance programs, eligibility requirements and benefits;
  • skills in case advocacy;
  • use of community resources and knowledge of the community service delivery systems and
  • specific skills necessary to serve our target population.

4. Excellent written and verbal communication skills and relational skills

5. Bilingual skills are highly desirable.

6. Ability to function as a team member, with initiative and flexibility.

7. Organizational skills, with good attention to detail and the ability to handle multiple responsibilities using appropriate time management.

8. Computer literacy and solid computer skills.

9. Possess and maintain a valid Driver’s License.

10. Ability to drive for work-related tasks.

Education and Experience Requirements:

1. HS Diploma/GED Equivalent, plus a minimum of four (4) years of experience working in individual, family and children’s behavioral health services and experience working with abused, neglected and traumatized children, adults and/or families.

OR

2. Associate’s degree in social services, plus a minimum of two (2) years of experience working in individual, family and children’s behavioral health services and experience working with abused, neglected, and traumatized children, adults and/or families.

OR

3. Bachelor’s degree or master’s degree in social services with minimum of one (1) year post graduate experience working in individual, family and children’s behavioral health services and experience working with abused, neglected, and traumatized children, adults and/or families.
