Duties:
As the senior Dietitian in Health Promotion (HP), assesses, analyzes, and determines nutrition needs for the base population using evidenced-based epidemiological principles, and uses evidenced-based nutrition education and behavior change theories to develop, budget, coordinate, implement, market, and evaluate comprehensive HP and installation nutrition and other health promotion programs. With a goal of nutrition improvement and disease prevention/intervention for the beneficiary population, assesses, analyzes, and evaluates the nutrition education and/or intervention needs. Employee uses judgment in interpreting and adapting guidelines, analyzes results, and recommends changes. Applies knowledge to difficult and complex work assignments, independently interprets policy in terms of established objectives, and resolves most conflicts as they arise. Provides recommendations based on conventional processes to improve nutrition program outcomes (individual, worksite, community, and/or policy). Performs the Community Needs Assessment, selects and uses other assessment tools based upon community needs and established goals and priorities. Uses health promotion strategic plan assessment results and installation population health priorities to develop and plan comprehensive, integrated, and effective primary, secondary, and tertiary nutrition programs and services. Evaluates unusual circumstances, variations in approach, and incomplete or conflicting data to determine or refine methods and techniques. Actively plans, implements, and coordinates installation working groups to affect change in the nutrition environment. Guides advocacy efforts to include identifying and including key influencers, leveraging community resources, selecting best practices, and implementing best practices to effect desired change. Executes Health Promotion initiatives, and leads and collaborates initiatives with key population health planners (i.e., Health Care Integrator, Group Practice Manager, and Population Health Working Group members). Develops, coordinates, and markets programs to target populations and health care delivery system for optimal participation and improved health outcomes. Delivers targeted prevention nutrition programs through individual and group education, lectures, workshops, and social marketing campaigns. Selects and implements the most appropriate behavior modification approach to optimize behavior change. Tracks program outcomes and client satisfaction to assess program effectiveness on nutrition practices, disease prevention, and overall health outcomes; implements targeted program or process change based upon deficits between observed and desired outcomes. Advises the Child Development Center and School Age Programs and, at a minimum, evaluates menus according to the United States Department of Agriculture (USDA) school lunch program guidelines.

Provides Medical Nutrition Therapy (MNT) for clients referred from the Medical Treatment Facility (MTF) and establishes nutrition-related chronic disease management protocols. Accepts and responds to referrals from MTF and network providers to counsel clients on healthy lifestyle behaviors to minimize health risks and prevent further disease complications from diagnosed disease processes. In conjunction with the Health Promotion Manager (HPM) and key population health planners, assesses the need for chronic disease management protocols and programs for the MTF enrolled population. Provides medical nutrition therapy and other nutrition care services to outpatients whose disorders/diseases may require complex dietary management. Adopts and implements appropriate clinical practice guidelines and protocols and coordinates implementation, integration, and process improvement with Primary Care Elements and Specialty Care Clinics, as appropriate. Interviews patients to obtain nutrition assessment data and information regarding usual dietary patterns, lifestyle, food purchasing practices and resources, and ability to understand and adopt a modified diet. Uses recognized assessment and evaluation techniques and tools that consider the varied needs of age-specific populations as well as cultural, religious, and ethnic preferences. Obtains and interprets timely anthropometric and biochemical data, and interprets data using evidence-based standards. Develops, implements, and individualizes plans for nutrition intervention in accordance with the client’s medical/personal goals, diagnosis, and self-management potential. Assists clients in developing realistic long-term health goals and goal-directed behaviors. Evaluates the effectiveness and measures progress towards outcomes of interventions and adjusts the nutrition care plan as indicated. Maintains ongoing communication with the patient’s referring provider and ancillary providers to ensure continuity of care. Provides appropriate documentation that summarizes the nutrition care plan and progress demonstrating accountability for the proper use of each client’s protected health information. Serves on interdisciplinary treatment teams to review patient’s medical backgrounds, psycho-social history, current dietary adherence, and method of administration of nutrition. Conducts patient education, both on an individual basis and through patient classes. Teaches diet principles and normal nutrition principles. Develops or modifies teaching materials. Independently plans, schedules, and provides comprehensive nutritional care on own initiative.

Serves as a nutrition consultant and advocates a comprehensive AF Nutrition Fitness Program for the installation. Serves as the professional nutrition consultant to commanders, first sergeants, and supervisors. Provides and oversees nutrition education and intervention to support AF Nutrition Fitness Program policy and meet the needs of installation agencies/units and Geographically Separated Units. Facilitates environmental assessment and community-based education/intervention (e.g., healthy snacks in vending machines, healthy choices at base dining facilities) to encourage and support healthy eating practices, weight gain prevention, and weight loss maintenance initiatives. Evaluates plans and treatment outcomes to ensure optimal health objectives. Participates in continuing education programs to update and/or maintain skills and knowledge, and conducts training of peers and subordinates. Upon request, leads in-services meetings/briefings and may participate in nutrition-related research projects. Maintains currency in wing-established and AF required training topics. Participates in and provides training to other dietitians, diet technicians, and food service workers. Provides in-service education training to outpatient personnel and other wing staff, as requested. Provides training to other health educators on general nutrition principles to help improve access and quality of care. Recommends, reviews, evaluates, and develops instructional materials for one on one, worksite, and community population nutrition education.

Adheres to sound business practices, collects outcome data, demonstrates process improvement functions, and participates in peer review process. Practices effective time management and prioritizing of tasks. Prepares and maintains records of appointments for scheduling and billing according to the latest version of Military Health Systems (MHS) Coding Guidelines, Diagnosis Coding (such as International Classification of Diseases, 10th revision (ICD-10)), Evaluation and Management (E&M) Coding, and Current Procedural Terminology (CPT) Procedures Coding. Provides completed documentation in required A.D.I.M.E. (Assessment, Diagnosis, Intervention, Monitoring and Evaluation) format for peer review as outlined in AF guidance to the Command Consultant Dietitian. Collaborates and supports MAJCOM RD requirements for peer review support, data calls, and monthly updates. Records time spent in tasks to meet Medical Expense and Performance Reporting System (MEPRS) requirements, enters patient encounters, and codes visits IAW AF guidelines to accurately capture productivity. Requests and manages resources in the most cost-effective manner. Identifies opportunities to increase medical staff, client, and community awareness of programs and services. Collects, records, and evaluates data from implemented client and population programs, reports to appropriate base agencies, and provides timely responses to data calls generated by AFMOA/AFMSA. Makes recommendations based on outcome data on how best to improve nutrition health and community nutrition environment. Supports and promotes the organization’s infection control, safety, risk management, and customer service programs. Participates in strategic planning to develop measurable goals and objectives. Leads or participates in departmental, organizational, and installation meetings, task forces, and projects. Participates in flight’s/squadron’s in-services, meetings/briefings, and process improvement activities.

Additional Responsibilities:
1. At a minimum, qualification requirements stipulated by the US Office of Personnel Management for the 0630 occupational series must be met.

2. A Master’s Degree in nutrition, nutrition education, health education, or health promotion demonstrating positive evidence of ability to perform all duties of the position, as well as 2 years’ experience, is highly desired.

3. Present or previous AF Health Promotion experience is highly desired.

4. Must accomplish the minimum Continuing Education Units (CEU), and be privileged through the Credentials Office, IAW Agency guidelines.

5. Membership in the Academy of Nutrition and Dietetics (AND) is highly desired.

6. Employee will be required to attend the AF Health Promotion Course and obtain the Academy of Nutrition and Dietetics certificate of training in Weight Management. Advanced certification in other areas, such as Board Certified Specialist in Sports Dietetics (CSSD), Certified Diabetes Educator (CDE), or Certified Health Education Specialist (CHES) is highly desired.

7. Employee is required to maintain status as a provider in accordance with the requirements of the national regulating agency, AF, and the MDG. Professional certification and currency must be maintained in accordance with AF and national policies.

8. The employee is required to keep abreast of changes in information, concepts, improvements, and requirements in the changing field of sports nutrition, community nutrition, health promotion, and medical nutrition therapy by attending conferences, attending medical meetings, and regular reading of appropriate professional journals.

9. Must possess and maintain current Basic Life Support (BLS) certification.

10. Work may require travel away from the normal duty station on military or commercial aircraft.

11. The employee may be required to work other than normal duty hours, which may include evenings, weekends, and/or holidays.

12. Incumbent must maintain a security clearance.
