JOB SUMMARY
This position is responsible for providing quality services to individuals dealing with the effects of addiction/behavioral health issues, who are at risk behaviors or are an individual who lives with HIV. This position will ensure that quality care outcomes are achieved through navigating the individual through community and outpatient medical options resources.
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Conducts intakes and assessments on new, existing and discharged persons served according to program requirements; i.e. meet quota according to guidelines.
- Develops and implements individual service plans to ensure individuals are receiving quality services according to service plan.
- Meet in secure, comfortable meeting places outside of office, as needed.
- Completes documentation as required by VOASELA and regulatory agencies/funding sources.
- Ensures that quality outcomes are achieved.
- Serves as advocate for individuals receiving services.
- Prepares accurate and timely documentation for billing of services.
- Keeps supervisory staff apprised of gaps in services and assists in efforts to provide needed services.
- Participates in program related training, as required.
- Become a Certified HIV/HEP C testing Counselor and perform functions and duties of that program.
- Facilitates meetings or providing formal peer support groups focusing on empowerment, recovery, and self-advocacy.
- Performs related duties as assigned.
KNOWLEDGE, SKILLS AND ABILITIES
This position requires the incumbent to have completed education at the Bachelor's degree level in a human service field or a Master s level preferred working in this field and one year care coordination or substance use experience and experience with working in a community setting; have a demonstrated willingness to be responsive to the needs of the target population, excellent verbal and written communication skills and the ability to function as part of a team; can handle a flexible work environment; have strong decision making skills.
