Responsible for controlling the release of information from the patient medical records in accordance with regulatory requirements, hospital policies and procedures in addition to Federal and State statutes including Health Insurance Portability and Accountability Act (HIPAA) guidelines.

Overview of Responsibilities:
  • Reviews and analyzes electronic and imaged medical records to ensure that required reports are included and signatures are not missing from the record.
  • Assigns chart completion to the appropriate providers through Epic.
  • Understands and accurately utilizes the deficiency tracking module of the hospital information system.
  • Facilitates medical record completion by generating and distributing record deficiency listings and compiling routine administrative and medical staff reports to support enforcement of medical record completion for both Reid Health (Richmond and Connersville) and Reid Health Physician Associates.
  • Assists medical staff and other ancillary departments by handling the retrieval of the electronic health record upon request.
  • An integral part of the Discharge Not Final Coded process in ensuring that all charts are completed and coded in a timely manner. Assists with getting the charts coded and sent to Patient Financial Services for billing purposes. Works with various hospital departments as well as the Director of HIM, Coding Manager, Operations Manager, etc.
  • Responsible for all release of information activities for the organization for audits, attorneys, disabilities, workman’s compensation claims, and insurance claims and reviews requests for patient information to ensure proper authorization for release of information following HIPAA regulations.
  • Works with all organization personnel involved with any aspect of release of confidential information, to ensure full coordination and cooperation under the organization’s policies and procedures and legal requirements.
  • Assists with handling walk-in requestors needing medical information.
  • Maintains monthly quality checks for timeliness of filling release of information requests.
  • Prepares and documents invoicing for requests requiring payment. Follows established procedures for the collection and handling of payments received.
  • Responsible for certifying all correspondence as requested.
  • Is knowledgeable regarding release of information guidelines associated with the release of information for psychiatric and substance abuse patient records.
  • Establishes processes to secure the medical record in potential cases of litigation.
  • Keeps abreast of changes in the release of information regulations at both the state and federal level. Implements changes to policies and procedures as appropriate.
  • Maintains accuracy and productivity.
  • Follows hospital and departmental policies and procedures.
  • Maintains neat, organized work area with adequate supplies.
  • Attends or views department meetings minutes and in-service programs.
  • Performs other duties as assigned.
Education/Experience:
Education Required: High School Diploma
Education Preferred:
  • Completion of medical terminology course, preferred.
  • Associate’s degree in health information technology or other healthcare related field.
Experience Preferred:
  • One to two years of clerical experience in a health information management department or clinical setting.

  • Experience and knowledge in information privacy laws, access, release of information, and release control technologies and the ability to accurately communicate information to patients, insurance companies, attorneys or physician offices the guidelines.
Licensure: None.
Certifications: Registered Health Information Technician (RHIT), preferred.
Other:
  • Demonstrated organization and communicate skills.
  • Strong computer knowledge.
  • Must demonstrate the ability to be detail-oriented and perform tasks at a high rate of accuracy.
  • Must be able to prioritize and organize activities in a fast paced environment.
Schedule Details: Day Shift. 40 hours weekly. Monday- Friday. 8:30am to 5pm. What We Offer:
