Segue Inc. is looking for full-time, caring, tenacious individuals who genuinely want to make a difference in the lives of our clients who deal with severe mental illness. The Mental Health Advocate is a vital member of the Assertive Community Treatment (also known as ACT) program. Our Advocates work with adults with a history of severe mental illness who may be at risk for inpatient psychiatric hospitalization or other intensive psychiatric services but can remain safely in their communities with an Advocate's support.

What Your Day Looks Like


You will meet each morning with your other multi-disciplinary team to provide services consistent with the ACT model ACT team to discuss the previous day's events and what is on the agenda for the day. Then it's off to see your clients! You may meet them in their homes, in a park, wherever they are, you'll be there. You may be delivering medications to them, talking about how things are going, you may take them to appointments, or helping them find community resources such as housing, entitlements, employment, education, vocational interests, community integration, and connection to physical health care.

Whatever is part of their treatment plan, you will assist in providing. Wellness and behavioral health recovery aim to maximize independence and progress into less intensive services.


You will return to the office at the end of your work day, type progress notes into our electronic medical system (EMR), and close out any outstanding paperwork.


Your schedule with be Monday through Friday from 8:00 AM to 4:30 PM. All team members rotate after-hours crisis intervention (on-call).


What is Required to Become a Mental Health Advice

MUST possess a verifiable Bachelor's degree in Social Work, Human Services, Criminal Justice, or Human Services or are a licensed RN to be considered,


AND

  • Be a Qualified Mental Health Professional (QMHP) with specialized training or one-year experience treating or working with a person with mental illness.
  • Having the ability to type a minimum of 35 wpm is also required. Experience entering detailed data in an electronic medical record system is preferred.
  • Must have a valid driver's license, vehicle insurance, and reliable transportation,


Other Responsibilities Include


  • Providing the expected number of services to clients each month, 80% of which are in the community
  • Working collaboratively with clients, their families, and community partners to assist clients in achieving their treatment and recovery goals.
  • Coordinating client care through close involvement with hospitals, substance use treatment facilities, etc.
  • Completing immediate needs assessments, comprehensive psychosocial evaluations, individualized service plans, and crisis/relapse prevention plans
  • Conducting specialized clinical assessments of violence risk, suicide risk, substance use, trauma, and criminogenic needs.
  • Using evidence-based treatments in the areas of mental health and substance abuse to address client needs (e.g., harm reduction, motivational interviewing, trauma-informed care)
  • Providing services to assist clients with needs such as family support, housing, entitlements, employment, education, vocational interests, community integration, and connection to physical health care.
  • Providing services to assist clients with legal advocacy, navigating the criminal justice system, and coordinating with legal agencies.


Compensation and Benefits


The rate of pay is $17.00 plus bi-weekly pay incentives!
