The Family Support Worker provides direct care services to youth with a case plan goal of APPLA to assist them in life skills development and assist them in accessing community resources and services to meet their individual needs and promote a successful transition from foster care.

Ø Mandated reporter.

Ø Must be able to manage aggressive behaviors with clients.

Ø Ability to work flexible schedule including on-call.

Ø Ability to engage youth in working relationships and assist them in developing positive connections in the community

Ø Understanding of child and adolescent development.

Ø Demonstrate effective written and verbal communication skills.

Ø Ability to travel throughout the State.

Ø Understanding of family centered practice, genograms, and ecomaps.

Ø Must be a team player.

Ø Handles multiple priorities.

Ø Independent discretion/decision making within the scope and responsibility of the position.

Ø Manages emotions and is able to make decisions under pressure.

Ø Manages stress appropriately.

Ø Works alone effectively.

Ø Provides direct services to assist them in life skills development and assist them in accessing community resources and services to meet their individual needs and promote their successful transition after foster care

Ø Support case plan goals and transition plans through teaching, role modeling, and demonstration.

Ø Participates in case conferences, client assessments, and in the team-decision making process.

Ø Supervises and transports youth, as needed.

Ø Assists youth to establish a support network with family, kinship, and community that will be available to the youth post custody

Ø Refers youth to community services and assists in arranging appointments as needed.

Ø Assists with all assessments and transition planning for the youth

Ø Provides assistance in crisis situations.

Ø Assists in ensuring the youth has their important documents such as birth certificate, social security card, state ID

Ø May assist with completion of aged out medical card application for youth

Ø Provides support to youth during this transition period after foster care


Ø Completes and maintains written documentation in youth’s case record accurately and within established time frames.

Ø Participates in supervisory and team meetings, sharing pertinent information about youth’s progress, collateral contacts and relationships with other agencies/communities.

Ø Works in partnership with other internal staff and community partners engaged with the youthØ

Ø Reports critical incidents to SRS verbally within 12 hours, written within 24 hours.

Ø Reports unusual incidents through appropriate Risk Management, clinical and safety channels.

Ø Implements SFCS policies and follows directives as required. Follows and adheres to all pertinent SFCS Standard Operating Procedures (SOP’s), rules, personnel policies and procedures; related accreditation and licensure standards; and federal, state and local rules.

Ø Ensures clients’ rights are protected.

Ø Is knowledgeable of and follows all safety procedures.
