FULL-TIME: 40 hours, Nonexempt

Coordinated Care Services Inc (CCSI) seeks a talented Family Access Facilitator to contribute their expertise in providing essential business services to our provider organizations and customers. As a Family Access Facilitator, you would be working as a member of the Family Access and Connection (Team) that serves as Monroe County’s entry point for families in crisis, or in need of assistance. The Family Access Facilitator plays a crucial role in the community by engaging at-risk families in exploring and evaluating their own unique needs and challenges. Through your support, intervention, and collaboration with community services, you would lead them to a healthier, more secure life.

CCSI is partnering with Monroe County Department of Human Services, Office of Mental Health and Probation to hire a Family Access Facilitator. This position is employed by CCSI and working on behalf of Monroe County.


As the
Family Access Facilitator, you will be

  • Conducting strength-based and cross-system assessments on assigned youth and families.
  • Responding to and creating crisis plans for youth and family crisis situations and accessing emergency services, as necessary.
  • Creating and facilitating individual plans of care based on a family’s strengths and needs.
  • Building effective connections and developing relationships with informal and community supports, child and family-serving systems, schools, and other formal supports.
  • Providing casework services to the youth and their family to stabilize the family unit and link them to appropriate supports and services.
  • Providing families with recommendations and referrals to appropriate support services, programs, and interventions.
  • Providing coverage for intake line and walk-in response for families initiating contact with FACT on a rotating basis
  • Other duties as assigned.
Requirements:

EDUCATION AND EXPERIENCE:

  • Bachelor’s degree preferred or equivalent education and work experience (see CCSI's equivalency chart).
  • A minimum of 1 – 2 years working with children and families is preferred.
  • Bi-lingual (English/Spanish) candidates are encouraged to apply.


Additional Requirements

  • Satisfactory completion of all required background screenings.
