Job Description
Responsible for directing daily operations of the Care Coordination departments with responsibility for both inpatient and outpatient servicesaThe Director Care Coordination is responsible for directing daily operations of the Care Coordination departments such as Care Coordination/Patient Support, Integrated Care and Palliative Care, and Utilization Management with responsibility for both inpatient and outpatient services. The Director provides effective leadership and works closely with physicians, department staff as well as managers and staff of other departments to guide operational direction, planning and coordination of the department services. Cultivates and maintains strong partnerships with post acute care providers including evaluation of post-acute networks, assessing post-acute network needs and coordinating educational resources to increase competency of post acute providers. Responsible for improving customer satisfaction and achieving the department goals, objectives and financial performance. Provides responsible resource/fiscal management and oversees continuous productivity and process improvement. Responsible for staffing, project planning/development, facilities planning, budget preparation, regulatory compliance, equipment selection and maintenance, supply ordering, and contract negotiation with vendors. The Director supports and promotes efficient throughput across the continuum resulting in optimal clinical outcomes, appropriate utilization of resources and accurate reimbursement. The Director is responsible for staff recruitment, retention, development, training and discipline. The Director also is an integral member of the readmissions prevention team as well as the length of stay improvement team.
QUALIFICATIONS:
Bachelor's in Nursing (BSN); Master's degree in Nursing, preferred
Five (5) years of experience with Care Coordination.
Three (3) years progressive management experience and ability to demonstrate competency in management skills.
Demonstrated leadership and excellent communication skills.
Must demonstrate ability to work well with patients/families, nursing and ancillary staff, and physicians.
Knowledge and expertise in utilization management and discharge planning and Medicare rules.
Demonstrated ability to work effectively under multiple demands, expectations and situations.
License/Certification/Registration Requirements
Current California RN license.
Certification in Case Management preferred
Salary Range:
84.45 - 126.67 USD Hourly
