We are hiring for our group homes loacted in Brooklyn Park and Hopkins.

We are currently hiring for all shifts part-time and full-time Direct Support Professionals, Group Home Managers, and a Designated Manager

For full-time and part-time work, we have positions that can work with your schedule

Sign On Bonus $250 after 3 months for full-time staff and $150 after 3 months for part-time staff.

Position Purpose:

Provide support, assistance and training to consumers in order that each consumer achieves their objectives and the goals. You will work with a variety of consumers in varied locations.

Major Job Function:

1. Provide support and training services to consumer in accordance with case plan:

a. Teach basic living skills

b. Teach household management/ budgeting skills

c. Provide transportation to families needing assistance to provide for their basic needs

d. Advocate for consumers

e. Assist consumers in finding low cost or free community resources

f. Teach basic health and safety skills

g. Implement crisis intervention procedures for consumers

2. Document and record family progress and services delivered

a. Maintain case notes

b. Contact and update program director and house manager

c. Keep accurate records of all contact with consumers and services delivered

e. Participate in evaluation of each progress toward it goals and objectives

3. Coordinate service strategies with team:

a. Communicate with other team members both services activities and concerns regarding each consumer

b. Participate in the development of case plans a strategy for services delivery

c. Assist program participants to identify concerns/ goals

d. Explore new community resources as assigned

4. Implement program and agency policies and procedures:

a. Protect data privacy rights of all consumers

b. Follow all laws governing the reporting of abuse and neglect of vulnerable adult

c. Complete administrative tasks on time

d. Follow New Hope Living Inc. & Nursing Services personal policies and procedures

Desired Applicant Should meet or exceed the following qualifications:

1. MUST be able to pass a state and federal background check

2.Have a high school diploma or higher

5. Have a valid driver license/ Clean driving record

6. Be able to navigate * the twin’s cities metro and surrounding areas

*Notes: this job may require bringing clients to appointments, and community outings.
