Remote with in county travel 4-5 times a month

COMMUNITY HEALTH JOB SUMMARY:

  • Develop county coalitions that address immediate needs and future opportunities.
  • Help community members access basic needs like food, PPE, mental health services, while providing updated information on mitigation strategies.
  • Assess performance of public health activities and identify and recommend methods for improvement.
  • Oversee, revise, and approve reports describing program outcomes.
  • Prepare, oversee, or finalize grant applications, program funding requests, and narrative reports.
  • Evaluate program achievements and identifies service gaps.
  • Assess the underlying causes of health disparities so that targeted interventions can be developed and implemented.
  • Recommend public health solutions such as screening, prevention, or intervention to address the social determinants underlying health disparities.
  • Develop initiatives with community partners to prevent or control public health threats.
  • Work with community partners to identify vaccination sites, promote vaccination clinics, identify trusted messengers to promote vaccination and prevention tactics.

COMMUNITY HEALTH HIGHLIGHTS:

  • Work schedule: Mon-Fri, 8a-5p
  • Travel required once per week to local clinics and county health departments in Buck or Montgomery Counties- Mileage reimbursement at current IRS rate.
  • Work from Home when not in the field
  • Community Activist or Public Health Educator with understanding of social determinants of health
  • Individuals without experience must be college educated and have experience through community service activities.
  • Full benefits available after 90 days: Medical, Dental, Vision, Life, Short-term Disability

  • The salary for this position is $41,600k

COMMUNITY HEALTH QUALIFICATIONS:

  • Bachelor's degree and two years of professional experience (or relevant community development or activist experience) in the development, analysis, or monitoring of programs, grants, or contracts in health, human services, social or behavioral services, health care services, or health insurance.
  • Knowledge of non-professional health care services designed to improve the ability of individuals to perform daily living activities
  • Comfortable traveling to local businesses, clinics and health departments Columbia and Montour counties
  • Bilingual Preferred
