Clinical Pharmacist specializing in Infectious Diseases and Antimicrobial Stewardship. This position has leadership responsibilities for antimicrobial stewardship efforts at Sinai Hospital and across the LifeBridge Health System.

• The ID/ASP pharmacist has patient care responsibilities at Sinai Hospital including, surveillance using TheraDoc and AcceleratePheno, approval of restricted agents
• ID/ASP specific decision support, tools, resources and protocols include:
  • InSightRx for AUC guided dosing per pharmacy driven protocol
  • TheraDoc for surveillance
  • Sanford Guide with integrated site/system specific guidelines and antibiograms
  • Restriction approvals, renal dosing, TDM, IV to PO by protocol
• The clinical pharmacist works closely with ID specialists, Infection Prevention, Microbiology Lab, medication safety, informaticists, nursing, hospitalists and other providers
• System and leadership responsibilities include:
  • Leading the ASP pharmacy service line for LifeBridge Health
  • Co-chair of the interdisciplinary Antimicrobial Stewardship Committee for LifeBridge Health
  • Presenting defined ASP metrics to Quality and Administration
  • Maintaining compliance and supportive documentation for regulatory readiness

Responsibilities also include areas of formulary management, drug information, practice advancement, process development and coordination of associated EHR enhancements. The successful candidate will have opportunities to serve as a mentor and educator for pharmacy residents, pharmacy students, pharmacy staff and healthcare professionals across other disciplines.

Provides direct supervision of Clinical Pharmacists and non-licensed Pharmacy Staff

JOB REQUIREMENTS:

• PGY-1 + PGY-2 residency in area of specialty + board certification or PGY-1 residency plus 2 additional years of direct patient care experience + board certification, or four post-graduate practice yrs of direct patient care experience + board certifIcation
• 3-5 years of experience
• BPS Board Certification required or equivalent
• Registered Pharmacist in Maryland
• Professional membership required
