Position Summary: You are invited to join the VNA hospice team as a clinical manager. In this role you are responsible for providing ongoing guidance and direction to the nursing team, in collaboration with the interdisciplinary team, for patients with life-limiting or terminal illnesses. Essential Functions: Ensures the provision of quality nursing service by assigned staff. Provides ongoing direction, guidance and evaluation of hospice nurses and insures that professional standards are maintained. Assesses requests for hospice service and initiates measures to provide for meeting identified service needs.

Other Duties:

  • Evaluates the level of competency and general professional performance of their staff and conducts supervisory visits in the field.
  • Identifies the in-service educational needs of staff individually and collectively and participates in planning and implementing appropriate orientation.
  • Directs and guides staff in their professional development in performing job responsibilities.
  • Maintains, improves, and applies knowledge of principles, practices, and techniques of management.
  • Analyzes the caseload to insure provision of appropriate and adequate service and to determine staffing needs.
  • Maintains clinical skills and professional nursing competency.
  • Assumes responsibility for professional growth and continuing education.
  • Establishes and maintains effective working relationships with staff, peers, supervisors and community agencies.
  • Maintains effective communication among peers to facilitate maximum daily coverage to all patients.
  • May be required to provide patient care in the field in times of organizational need.
  • Provides administrative back-up for evening and weekend staff. Supports on-call triage staff to ensure adequate on call coverage with schedules.
  • Directs and guides staff in caseload management and establishing priorities.
  • Insures that assigned nursing staff communicates with other Inter-disciplinary team members to facilitate coordination of patient care.
  • in-service programs in cooperation with Education and Quality leadership.
  • Completes all workflow tasks assigned in Home Care Home Base
  • Insures documentation is current and accurate and in keeping with hospice policies.
  • Participates in the educational process of students.
  • Interprets hospice service & programs to patients, families, community and other agencies.
  • Participates in Quality Assessment and Performance Improvement activities.
  • Participates in planning, implementing and evaluating hospice services and programs.
  • Complies with Agency's Personnel Policies and Procedures.
  • Performs other related duties as assigned.

Knowledge and Experience:

  • Requires hospice care experience.
  • Current Pennsylvania RN License

Education: BSN prefered
