  • Provide person-centered services to help persons served achieve expressed goals
  • Provide regular visits with persons served in the community and elsewhere to per treatment plan and client schedule
  • Write, implement, and monitor Individualized Treatment Plans with persons served within due dates outlined by agency guidelines
  • Coordinate with HCH on individual’s SMART goals and other HCH services and actions
  • Monitor the medical and psychiatric treatment of persons served to assure effective coordination and continuity of care
  • Provide 50% of each work day to appropriate service visits with person’s served per treatment plan
  • Complete and report service activities accurately, comprehensively, and within required standards outlined per agency standards of 5 business days
  • Facilitate self-sufficiency of persons served and maximize independence in the community; link and refer to community resources as appropriate
  • Complete assigned DLA-20 functional assessments quarterly and, when necessary, make appropriate contacts for change in level or type of service needed
  • Maintain high quality and achievement of targeted results through active participation in performance management and the principles and practice of CQI
  • Keep current on agency initiatives/procedures/policies through all forms of communication (email, intranet site, newsletters, etc.)
  • Provide crisis intervention and resolution for persons served as necessary
  • Maintain confidentially of persons served as outlined by agency policy
  • Identify issues impacting abilities of persons served to function optimally; implement solutions or coordinate with other resources to resolve problems
  • Positively contribute to the treatment team and attend team meetings as scheduled by team leaders
  • When assigned, provide on-call coverage during the hours when the agency office is closed
  • Provide support beyond the normal 40 hours depending on the needs of the team and caseload as appropriate and approved by supervisor
  • Effectively manage time and adjust to changes within the regular schedule and throughout the day to assure positive client outcomes
  • Assist persons served in meeting vocational goals; when necessary, work as a job coach, link clients with job training and placement programs, teach job finding and retention skills, and provide support for persons served on the job
  • Complete required training hours as outlined by employee handbook, and maintain any licensure requirements and provide timely documentation to HR Office
  • Fulfill on-call requirements as assigned
  • Participate in professional development annual planning to increase opportunities for career advancement
  • Perform other duties as assigned by the supervisor
  • High school degree and four years of relevant experience providing services to individuals with mental illness, developmental disabilities, and/or substance abuse.
  • Bachelor’s Degree in a social services field preferred.
  • Combination of education and experience may be accepted.
  • CPR, First Aid, and Medication Administration certifications required.
  • Chauffeur’s License, personal vehicle, and valid auto insurance required.

The Integrated Health Coach Specialist provides disease management and skill promotion services directly to persons served and/or provides linkage or referral to appropriate community resources. Provide support and education to persons served on mental health, substance abuse, physical health, and/or infectious disease management. Assist person served with addressing social determinants of health (e.g. – housing, transportation, community integration, etc.) Provide advocacy and crisis intervention as needed.
