La Posada Home, Inc.

The Case Manger for La Posada Home, Inc. will assure that program meets clients’ needs in order to successfully accomplish goals established in the Individual Service Plan. In addition to this, case manager guarantees that program provides a nurturing, supportive, healthy environment.

Duties

Case manager is in charge of:

 Intake
Answers phones for inquiries, sets up and conducts interviews, and staffs cases for admission with

Executive Director

 Development of Individualized Service Plan
Meets with clients once every week to assist them in establishing long-term, healthy goals for themselves and their families in order to promote self-reliance.

Develops, and regularly evaluates, the Individualized Service Plan in order to clarify for clients and for staff the plan to follow

 Conducting weekly meetings with clients to assure that ISP is being followed and to assess emerging needs.

 Conducting regular meetings with group for conflict resolution, clarification of rules, assure programs’ effectiveness, team building, and for crises management

 Assuring children’s needs are being met through meetings with parents, teachers, doctors, and other service agents.

 Celebrates birthdays, anniversaries, and other culturally appropriate holidays to strengthen nurturing and stabilizing traditions for families

 Arranging monthly groups meetings for clients’ self-improvement through agencies’ presentations, videos, conferences, and other resources.

 Disciplinary actions

 Maintenance of daily logs, entry and exit statistics, and other data for reports.

 Follow-up with clients 30, 60 and 90 days after release.

 Attending weekly supervisory meetings with Executive Director.

 Conducts street outreach to include: presentations, health fairs, and other events in order to reach out to the community.

Requirements:

* Bachelors of Social Work or other related Sciences

* Experience working with multicultural, at risk groups

* Bi-lingual

* Computer literate

Personal Skills

* Ability to maintain a professional, nurturing, honest relationship with clients and staff

* Ability to provide a role model for clients

* Ability to communicate clearly verbally and in writing with the public, clients, and staff

Job Type: Full-time

Pay: $35,000.00 - $37,000.00 per year
