This is a full-time telework role, open to candidates located in Maricopa County, AZ. 10-20% travel will be required to meet with clinical teams at community health homes and for meetings, as needed.

Working schedule: Monday-Friday, standard business hours 8am - 5pm AZ

Mercy Care RBHA care manager utilizes critical thinking and judgment to collaborate and inform the case management process, to facilitate appropriate healthcare outcomes for members by providing care coordination, support, and education for members through the use of care management tools and resources.

Mercy Care RBHA care manager utilizes knowledge of program requirements, network, and community resources to facilitate appropriate physical and behavioral healthcare and social services for members through collaboration with internal and external providers. Mercy Care RBHA care manager supports integrated care for the members, which centers on targeting social determinants of health concerns.

Pay Range
The typical pay range for this role is:
Minimum: 19.50
Maximum: 40.10

Please keep in mind that this range represents the pay range for all positions in the job grade within which this position falls. The actual salary offer will take into account a wide range of factors, including location.

Required Qualifications

• 2+ years of experience in behavioral health or social services
• 1+ year(s) of experience with Microsoft Office applications, including Excel, Outlook, and Word
• Travel will be required to meet with clinical teams at community health homes and for meetings, as needed.
• Candidate must reside within Maricopa County, AZ

Preferred Qualifications

• Experience working with the SMI population
• Case management experience
• Discharge planning experience
• Managed care experience

Education

• Bachelor's degree in a behavioral health field required
• Non-licensed Master's level clinician in a behavioral health field preferred
