CARE MANAGER
Location: Richmond Maine
Richmond Area Health Center is seeking a CARE MANAGER (32 hours/week) to join our active and friendly family practices. Responsibilities include Patient education, post-hospitalization follow up, chronic disease management support, and support for patients during care transitions, running and managing various reports identifying gaps in care and documenting medical information in the electronic health record. The candidate for this position should be team focused, organized, and have excellent communication.
All new hires will be required to provide either proof of COVID-19 vaccination or a recognized exemption.
