Summary:

Responsible for coordinating care across the continuum of care (inpatient/outpatient/community) to assure appropriate utilization of resources. Coordinates discharge/transition plan from inpatient or ED. Collaborates with the patient, family, health team members, and community resources to ensure safe, quality and cost effective care. The Care Coordinator assists patients to reach optimal function and health by establishing goals based on the patient’s needs or problems. Explores patient’s understanding and knowledge of current health status. Educates patients on medications, disease conditions and self-management skills. Uses assessment skills and risk assessment tools to identify patients with actual or potential health care needs that would require care coordination. Applies the process of assessment, planning, implementation, monitoring, evaluation and coordination of care to meet the patient’s health care need at all levels of transition. Willingness to cross train to all areas of care management.

Requirements:


Education, Training or Certifications Required:

1. Current Maine RN license.

2. Current BLS.

3. Bachelors degree preferred.

4. Evidence of continuing education in Care Management.

Experience Required:

1. Five years of recent healthcare experience in the acute or long term care setting.

2. Demonstrated knowledge of health, social service, and funding sources.

3. Experience in the provision of healthcare services to the population served.

Cognitive Requirements:

1. Able to communicate effectively in English, both verbally and in writing.

2. Additional languages preferred.

3. Strong interpersonal skills.

4. Good focus on standards and ethics.

Physical Demands:

1. Able to sit, stand and walk for an 8 hour shift.

2. Adequate hearing, vision, speech and manual dexterity.

3. General health must be such as to regularly work scheduled hours.

4. Able to lift and carry 25-50 lbs.

Environment:

1. Potential for exposure to blood and body fluids.

2. Potential for exposure to contagious diseases.

3. Potential for periods of high stress.

4. Frequent use of VDT and contact with third party payers
