Position Title: Care Coordinator/ Spanish Bilingual
Location(s): Remote/ Occasionally reports to office in Brooklyn NY
3-4 Months Contract
Case Load: 25+
Schedule: Monday - Friday 9am - 5pm
Hourly Rate: $22
- Mostly remote positions until further notice, Will need to meet with clients face to face and have a working laptop.
Requirements:
- Bachelor's Degree in Human Services or related degree
- Covid Vaccination Proof
- Minimum (2) years of experience providing direct services to clients suffering from mental health, substance abuse or homelessness
- Must have Laptop/ computer and Internet Connection
Responsibilities:
- Responsible for an individual caseload of 25-30 adult clients, each of which is to be visited one face-to-face and one telephonic once a month.
- Assure that clients attend medical appointments at outpatient clinics as well as comply with the medication regimens.
- Helps participants obtain benefits including food stamps, ongoing financial and housing needs, health care
- Initiate appropriate referrals to additional services including home care and other necessary referrals as needed.
- Facilitate and participate in client Case Conferences for medical/psychiatric and peer review.
- Immediately reports serious incidents, serious incident allegations, incidents. Completes incident reports
- Write clear Progress notes of services and visits in patient’s electronic medical record, closes encounters within 48 hours.
