Champlain Valley Family Center has 2 full time Youth Specialist positions available:

As a Youth Specialist, your outreach and support of children in the Intensive Family Support Program will help children and their families create service plans that will help them achieve success and through a strength based approach. The Youth Specialist will identify additional services and coordinate their involvement. A warm personality, and team-driven approach will add strength to this program and the families they serve.

Job responsibilities/duties:
* Assist Parents/Guardians with developing working behavior plans and modify and adjust them as necessary. Assure responsible transition from and to appropriate programs and services. Work collaboratively with schools, social services and other service providers as needed.
* Develop a comprehensive, family-driven youth empowered service plans. Inventory and coordinate with existing services. Identifying and securing additional services as appropriate. Provide extracurricular activities to the identified youth and families.
* Provide transportation of clients and family members. Assist clients to participate in various school, community and social activities. Maintain all paperwork required and keep up to date records.
Skills and Qualifications

• Bachelor’s degree in human services field with 2 years’ relevant experience or Master’s degree in human services field with 1 year of relevant experience
• Relevant experience is defined as serving children and families in child welfare, developmental disabilities, behavioral health, primary health care, or social services and understand the Person in need of supervision process.
• Excellent engagement and organizational skills in order to deliver family-driven, youth-guided services
• Ability to work independently and collaboratively
• Valid NYS driver’s license
• Experience coordinating and participating in team settings preferred
• Strong written and verbal communication skills
