Responsibilities:
- Observe, report, and document patient's status and the care or service furnished.
- Provide hands-on personal care as directed by the RN Manager.
- Demonstrate a positive attitude and working relationship with staff members, volunteers, patients, and families.
- Use appropriate and safe techniques in performing personal hygiene and grooming tasks.
- Maintain a clean, safe, and healthy environment.
- Complete records accurately and performing all assignments as requested.

Qualifications:
- Must have at least a High school diploma or equivalent.
- Must have experience as a CNA (strongly preferred but not required).
- Must have effective communication skills, including the ability to read, write, and verbally report clinical information to patients, caregivers, and other hospice staff.
- Must have excellent documentation skills and be able to understand, read, write and speak English.
- Must demonstrate organization, flexibility, and cooperation in performing job responsibilities.
