Job Title: Tribal Vocational Rehabilitation Case Manager

Reports To: Tribal Vocational Rehabilitation (TVR) Program Manager

General Functions:
The Tribal Vocational Rehabilitation Case Manager provides Vocational Rehabilitation (VR) Services to all eligible Alaska Natives and American Indians consistent with their individual strengths, resources, priorities, concerns, abilities, capabilities, and informed choices so that they may prepare for, or engage in gainful employment, including self-employment, telecommuting, or business ownership.

Duties and Responsibilities,* including but not limited to:

  • Conduct orientation sessions for the CITC-TVR program describing eligibility criteria for potential participants with disabilities.
  • Conduct interviews with potential participants; explain program services and requirements to all interested referrals and applicants.
  • Collect all required documentation and determine eligibility for each potential participant.
  • Develop an Individualized Plan for Employment (IPE) jointly with each participant.
  • Conduct or provide assessments and evaluations for TVR participants to determinate eligibility'services as outlined in the IPE.
  • Assist participants with supportive services according to program policy, procedure and budget.
  • Work as part of a cohesive team who recognize and understand their role within the larger departmental team.
  • Work as part of a team: the team includes co-workers, partner agencies and the participant.
  • Assist each participant in becoming active in his/her rehabilitation program, and aid them in making meaningful and informed choices about the selection of a vocational and/or other rehabilitation services.
  • Provide one-on-one coaching and guidance in job search and life management; provides problem-solving support in resolving issues that could result in job loss.
  • Utilize and refer participants to employment resources within CITC, especially Supported Work Experience, and Alaska’s People department to provide work and training opportunities for work ready participants.
  • Develop a knowledge base on community resources to assist participants achieve their vocational goals.
  • Participate in Integrated Case Management meetings for participants who are working with other internal/external agencies or natural supports.
  • Coordinate the delivery of program services with the Alaska Division of Vocational Rehabilitation and other Social Services agencies to maximize the availability of services to participants in an efficient, professional and timely manner.
  • Become familiar with Vocational Rehabilitative process and the numerical status involved to monitor client’s progress in their rehabilitation programs.
  • Participate in training and professional development activities as scheduled.
  • Maintain an active caseload and submit monthly, quarterly, and year-end performance reports as required.
  • Inform and consult with TVR Program Manager regarding program activities, issues and concerns to seek guidance and direction for the program as deemed necessary.
  • Responsible for current statistical database input on participant file activities (progress case notes, expenditure records, monthly status change records, IPE services summary etc.), and retrieving of monthly statistics for program reports.
  • Schedule assessments, evaluations and appointments for TVR participants as necessary to determine appropriate TVR services.
  • Process paperwork for program expenditures for Program Manager to approve.
  • Refer participants to ETSD Life Skills Classes as appropriate.
  • Provide referrals to Cook Inlet Tribal Council’s (CITC) services such as Recovery Services, Alaska Native Justice Center, Child and Family Services, 477 Youth Services, Youth Empowerment Services, Alaska’s People and other ETSD programs.
  • Provide eligible TVR participants with case management to assist in their rehabilitative needs.
  • Assist with the management of the CIRI region village caseloads as needed.
  • Exercise initiative in identifying community and individual need for services, and assist in research and development of community resources to meet such needs within a geographic, cultural, or program area.
  • Maintain confidentiality according to regulations, policies, and procedures.
  • Performs all related duties as needed and assigned.

Minimum Qualifications:*

  • Bachelor’s degree in Human Services or related field; or Associate’s degree with at least two (2) years’ experience in Vocational Rehabilitation may be substituted for bachelor’s degree requirement.
  • Two (2) years’ experience in Vocational Rehabilitation or closely related field.
  • Knowledge of federal and state laws, rule, and regulations governing vocational rehabilitation required Under AAC 54.600-630, Title VI of the Civil Rights Act of 1964 as amended, the Rehabilitation Act of 1973 as amended in 1992, and the Americans with Disabilities Act of 1990.
  • Basic knowledge of P.L. 102-477 Service Delivery System.
  • Continued employment is contingent upon receipt of a satisfactory report from a state and federal background check.
  • Valid Alaska driver’s license and insurable under CITC’s automotive insurance, which requires a driver to be at least 21 years of age and have had a driver’s license for at least three years.
