Schedule: Full-Time, 36 hours a week.

Flexible Schedule: Monday thru Thursday

Fixed schedule: Friday 8am - 5pm

The role you’ll contribute:

Triage & Education, reporting to the Physician, Mid-Level and/or Office Manager and while working within an outpatient office setting, provides triage (sorting/prioritizing patients) telephone service to ensure prompt identification of patients with high-risk conditions. May refer patients, schedule appointments or provide appropriate education/advice to callers. Additionally, may provide nursing care for patients at the practice.

  • Implements and monitors patient care plans. Monitors, records, and communicates patient condition as appropriate.
  • Serves as a primary coordinator of all disciplines for well-coordinated patient care.
  • Notes and carries out physician and nursing orders.
  • Assesses and coordinates patient's discharge planning needs with members of the healthcare team.

  • Excellent communication skills particularly listening.
  • Attention to detail.
  • Critical thinking skills and sound decision maker.
  • Proficient in computer, Microsoft office, EMR and telephone skills.
  • Familiar with standard concepts, practices, and procedures within a medical subspecialty

Qualifications


EDUCATION AND EXPERIENCE REQUIRED:

Diploma from an accredited school/college of nursing required.

One (1) to two (2) years of telephone triage experience, preferably in medical practice setting


LICENSURE, CERTIFICATION OR REGISTRATION REQUIRED:

  • BLS Provider credentialed from the American Heart Association (AHA) obtained prior to hire date or job transfer date.
  • Registered Nurse credentialed from the Illinois Board of Nursing obtained prior to hire date or job transfer date.
