The Transitional Services Manager’s main function is to focus on the development of New Freedom quality of care standards. The Transitional Services Manager will monitor staff performance metrics and promote accountability in meeting the metric standards with our supervisors. Working with the Transitional Services Supervisors and other departmental resources, they will ensure necessary training formats are developed and used in bringing New Freedom employees and processes to a level of excellence and efficiency. They will function as a role model to peers, exhibiting competency in personal re-entry and use of coping skills, serve as a consumer advocate, providing consumer information and transition support for members in outpatient and inpatient settings. Using their recovery or work-related experience, the Transitional Services Manager performs a wide range of tasks to assist the Transitional Services team with ensuring members master their re-entry process and effectively regain independence within the community. The Transitional Services Manager shall manage a team of highly motivated supervisors with the unified goal of successful and marketable employment skills development, job management, further developing quality of character, and member care.

Duties and Responsibilities:

  • Provides direct oversight of Transitional Services supervisory staff and the day-to-day operations of the Transitional Services department, by:
    • Providing ongoing guidance, direction, and accountability
    • Ensuring supervisory staff are monitoring and coaching staff with development and demonstration of the necessary skills and knowledge to perform their roles, and adherence to the company’s policies and procedures
    • Participating in and/or ensuring participation in staffings and shadowing assigned staff to monitor the quality of services provided
    • Ensuring accountability with daily staffing emails, quality of services provided, quality of documentation/billing within KIPU, and timely review and resolution of KIPU reports and data
    • Monitoring departmental performance metrics such as engagements, timely transitional tracker needs, clinical oversight and supervision, and timely review and sign-off of clinical documentation
    • Ensuring supervisory staff thoroughly document staff interactions within the appropriate system(s)
    • Ensuring proper collaboration and coordination of discharges, transfers, behavioral modifications, and notices to vacate
    • Assessing the member’s change in mood to determine and/or identify whether positive, negative, and/or potentially dangerous, taking the necessary steps to modify the member’s actions or alert the Clinical team as appropriate
  • Completes/attends, and/or provides ongoing training that is timely and designed to effectively contribute to successful accomplishment of department and company goals
  • Attends departmental, company, and/or community meetings as required
  • Completes other various duties/responsibilities, including but not limited to:
    • KIPU maintenance/monitoring
    • Staff scheduling
    • Timesheet and time off reviews, corrections, and approvals (payroll processing preparation)
    • Disciplinary action, annual performance reviews, and professional development of staff
    • Staff retention initiatives
    • Assists with management of recruitment, selection, hiring, onboarding, and training
    • Compliance with applicable contractual and/or state requirements, such as Arizona Administrative Code (AAC), Commission on Accreditation of Rehabilitation Facilities (CARF), and/or company policies and procedures
    • Maintenance of Transitional Services SharePoint information/content
  • Partners and collaborates with the Peer/Cultural Director in implementing and maintaining the desired team culture and team-building promotion, in addition to other department leaders and Human Resources
  • In addition, all other duties as assigned

Experience

Required
  • Excellent communication, organizational, time management, and critical thinking and problem-solving skills required
  • Behavioral Health Technician Certification and/or qualifying designation required
  • Associate’s Degree or higher in relevant field preferred (and/or equivalent combination of education/experience)
  • Valid AZ driver’s license required
  • Successful clearance of annual MVR required
  • Successful clearance of pre-employment criminal/county/state background check
  • Successful clearance of pre-employment drug and alcohol testing
  • Successful clearance of annual TB PPD testing
