Provides a wide variety of social work services to patients and their families. Working with all age populations.
JOB QUALIFICATIONS
Education: Masters degree in social work.
Licensure: Current Georgia License. Membership in the Georgia Chapter, National Association of Social Workers preferred.. Must have a valid drivers license and automobile insurance coverage in accordance with Hamilton Health Care System Driver Requirements policy.
Experience: Minimum graduate level internship in medical setting.
Skills: CPR License. Must maintain a high level of communication with associates, patients and medical staff. Requires clinical counseling skills as required by Medicare/Medicaid. Also the ability to offer higher counseling levels during crisis and suicidal events Completes psycho/social assessments at the start of care. Knowledge of organizational policies, regulations and procedures to administer patient care. Knowledge of medical equipment and instruments used to administer patient care. Knowledge of common safety hazards and precautions to establish a safe home environment. Skill in applying and modifying the principles, methods and techniques of professional social work to provide on-going patient care. Skill in identifying problems and recommending solutions. Skill in preparing and maintaining records, writing reports, and responding to correspondence. Skill in developing and maintaining department Performance Improvement. Ability to maintain quality control standards. Ability to react calmly and effectively in emergency situations. Ability to interpret, adapt and apply guidelines and procedures. Ability to work independently in patients homes without direct supervision. Ability to drive self to various locations on a daily basis.
