Givens Estates, a nationally recognized nonprofit life plan community (continuing care retirement community), is hiring a Social Worker I to join our Independent Living Life Enrichment team. This full-time and non-exempt position will help support our growing independent living population. The schedule that we are currently seeking is regular business hours with nights and weekends as necessary when on-call.

What you'll do:

  • Collaborate with other Independent Living Social Workers, the Givens Estates Health Center and Assisted Living Social Workers, Campus Nurse, Director of Nursing, and Director of Ministries
  • Implement and evaluate planned social services activities, both on group levels and in one-to-one settings
  • Provide referral and community resource information to residents or families as needed
  • Advocate for and uphold Residents' Rights, including documentation and follow-up on resident and/or family complaints or grievances
  • Monitor the needs of residents regularly and coordinate services or provide services to meet identified needs when appropriate
  • Work with the Campus Nurse, Director of Nursing and Administration in developing care plans tailored to the needs of individual residents, particularly as it relates to pre-and post-hospitalization
  • Develop supportive and caring relationships with residents and provide support and counsel residents during crisis/death of spouse and friends.
  • Foster a sense of Community among various residential options (rental units, villas, duplexes, cottages, houses, apartments, assisted living, and health center)
  • Play an active role in the new resident orientation process by working in collaboration with the Marketing team and Campus Nurse to assess, approve, and coordinate new residents' move-in and orientation to independent living

What you'll need:

  • Bachelor's degree with a concentration in a Human Service related field required
  • 2+ years of experience working with older adults
  • Ability to work nights and weekends as needed
  • Database, spreadsheet, and Microsoft Office experience
  • Working knowledge of the health care system and levels of care
  • Ability to work independently, proactively, and with teams as required
  • A valid NC Driver's License with a clean driving record for the use of a facility vehicle
  • Excellent communication, interpersonal and organizational skills
  • Ability to make individual and group presentations

Compensation: Hourly rate is competitive based on experience and includes a comprehensive benefits package!
