Position Summary

The mission is to provide inpatient mental health care, evaluation and treatment to adult clients in the LSH catchment area, the Department of Corrections and all District Courts of Kansas. Through collaboration with community based agencies, community based mental health providers, substance abuse treatment providers and legitimately involved others; services will be coordinated to provide quality social services to patients. Staff will participate in naturally occurring and/or assigned teams to impart job knowledge and skills to provide quality customer services.

Qualifications

Minimum

  • At the time of appointment, eligible candidates for this position must possess a Licensed Bachelor Level Social Worker (LBSW) license. Students currently enrolled in an accredited social work program who have applied to take the licensure exam will be considered.
  • Licensed by the Kansas Behavioral Sciences Board as a Bachelor’s Level Social Worker, Master’s Level Social Worker or Licensed Specialist Clinical Social Worker
  • Some positions require an approved drug test unless the incumbent is moving from one safety-sensitive position to another safety-sensitive position within the same agency performing substantially similar duties.
  • CPI certification

Tasks/Duties

Social Work Specialist Duties:

  • Make collateral contacts (i.e. courts, MHC’s, KDADS, CDDO’s, KDOC, Law Enforcement Agencies and legitimately involved others) to gather and share information pertinent to the patient’s treatment, maintaining professional standards of confidentiality.
  • Make initial contact with newly admitted patient(s) to identify and implement indicated social services.
  • Maintain regular contact with patient to assess progress toward identified treatment goals.
  • Develops and implements community based aftercare plans in collaboration with involved community agencies and legitimately involved others.
  • Assure patient’s rights have been provided to assigned patient(s).
  • Provide patient and/or family and/or legal guardian education specific to Social Work Specialist role and indicated treatment goals. Educate patients regarding psychiatric disorders, treatment, management of symptoms and self-care as indicated.
  • Explain legal status to patients as indicated.
  • Provide supportive counseling to patients, family members and/or legal guardian(s), as needed.
  • Provide court testimony as required.
  • The individual in this position has access to Protected Health Information (PHI) under the provisions of Health Insurance Portability and Accountability Act of 1996 (HIPPA). PHI must be safeguarded and released in accordance with the provisions of the HIPPA Privacy Regulations and state and federal law.

Multidisciplinary Team Duties:

  • Participate in multidisciplinary treatment team meetings and Clinical Executive Committee (CEC) Consults as indicated to assess patient’s treatment progress and identify treatment needs in compliance with LSH policies and procedures and State/Federal Mandates.
  • Provide social work input for court ordered evaluations/reports.
  • Share pertinent information vital to patient treatment and discharge.

Documentation:

  • Complete Psychosocial Assessments and Interim Psychosocial Assessments.
  • Document Social Services treatment course and progress in treatment in accordance with department and agency standards.
  • Document contacts with patient, any family contacts and/or guardian contacts and/or community agency contacts.
  • Document progress toward discharge and develop and complete Discharge Instructions.

Other Social Services Department Related Duties as Assigned:

  • Attend Social Services Department meetings, Program meetings, Unit Meetings and other meetings as scheduled/assigned.
  • Participate in Performance Improvement Projects as assigned.
  • Participate in committees as assigned.
  • Completes special projects as assigned.
  • Other duties as assigned by Supervisor, Department Head, and/or Program Director.
  • Attends Agency mandated and Social Services Department mandated training.

Other Duties as Assigned

  • Engage in continuing education to maintain current skills and competency and to keep abreast of current trends, broaden knowledge, learn new skills and to maintain licensure
  • Perform additional job related duties as assigned by supervisor and Director of Social Services to accomplish the mission and goals of LSH.
  • Completes special projects and performs other duties as assigned including but not limited to ensuring the agency is adequately staffed during and immediately following natural and/or manmade disasters, infectious disease outbreaks, and/or acts of terrorism. Assists other employees in the work unit in accomplishing assignments as necessary.
