JOB SUMMARY: The Director of Social Services is responsible for planning, organizing, staffing, directing, coordinating, reporting, budgeting, and physical management of the social service department's employees and equipment in order to implement group and one-to-one psycho-social programming as well as meet with interdisciplinary team members. The Director of Social Services reports directly to the Administrator.

JOB REQUIREMENTS:

1. Must be physically and mentally capable of performing routine job duties.

2. An understanding of the psycho-social dynamic of the geriatric population and the ability to empathize and provide guidance to them.

3. Ability to relate well to and communicate with residents, family members, and staff.

4. Good administrative, managerial, organizational, planning, and interpersonal skills.

5. Must have compassion, tolerance, and understanding for the elderly.

6. Knowledge of ODH regulations relevant to social services.

MAIN DUTIES:

A. Support the facility's philosophy of care and strive to achieve its goals and objectives.

K. Schedule group and one-to-one sessions to accommodate all appropriate residents.

L. Review monthly progress notes, quarterly assessments, and social histories and countersign them.

N. Complete the Psycho-Social Section of the MDS form upon admission, quarterly, annually and/or in cases of significant change and coordinate suitable programming for residents identified with psycho-social needs.

O. Track discharge and readmission of residents to determine the need for reassessment and inform the staff to follow up.

P. Identify and communicate with the Psycho-Social staff regarding any needed change in goals, program schedules, etc.

Q. Review Psycho-Social care plans for their resident appropriateness.

R. Attend admission, Care Plan, Medicare, Department Head, and Performance Improvement meetings and others as deemed appropriate.

S. Assist in the determination of appropriate room changes and speak to residents and family members when necessary.

T. Work with residents and families on room changes, complaints, and problems of adjustment.

U. Communicate with other discipline staff about psycho-social programs and resident programs.

V. Coordinate monthly family meetings and special programs for residents, families, and the community.

W. Inform families of any significant change in resident's psycho-social status and initiate adapted psycho-social program to address the present condition.

X. Coordinate all aspects of hospice care for those residents or families who choose hospice care.

Y. Coordinate discharge planning for appropriate residents.

a) complete the MDS discharge form.

b) contact families to arrange care giving and/or transportation.

c) contact home health agencies to give referral and provide adequate information to the intake nurse.

d) obtain through nursing the correct physician's orders and provide nursing with information regarding home health agency.

e) order appropriate equipment from DME provider in conjunction with physical and occupational therapy and the patient as to type, cost, and size.

f) document referral information and discharge disposition for the medical record.

g) make referrals to other social service agencies such as home

delivered meals and homemaker services as needed.

Z. Supervise documentation including:

a) schedule of program meetings' time and place.

b) list of one-to-one and group participants.

c) group protocols.

d) lesson plans.

AA. Maintain up-to-date knowledge of any changes in ODH, Federal, and State regulations, modify department policies to assure compliance, and in-service all effected personnel accordingly.

AB. Implement any plan of correction as required by State and Federal surveys in the psycho-social department.

AC. Furnish information regarding residents only to authorized agencies or people.

AD. Follow established fire, disaster, safety, infection control, and evacuation policies and procedures.

AE. Perform other related duties as directed by the Administrator.

AF. Work as weekend Supervisor as part of Department head monthly rotation.

Ascertain that each employee in the psycho-social department is aware of the lines of authority and exercise disciplinary action when necessary and provide appropriate documentation in the employee's personnel record.

H. Oversee the new employee orientation process including the explanation of personnel policies and individual duties regarding social services.

I. Supervise staff in leading groups and one-to-one sessions.

QUALIFICATIONS:

1. A Master’s degree in Social Service or related field is preferred. SIGN ON BONUS FOR LSW!

2. Two years of experience in the field of social work, preferably in a long-term care environment.

PHYSICAL AND SENSORY REQUIREMENTS:

  • Must be able to move intermittently throughout the workday
  • Must be able to speak and write the English Language in an understandable manner
  • Must be able to cope with the mental and emotional stress of the position
  • Must possess sight/hearing senses or use prosthetics that will enable these senses to function adequately so that the requirements of this position can be fully met
  • Must function independently, have flexibility, personal integrity, and the ability to work effectively with residents, personnel, and support agencies
  • Must meet the general health requirements set forth by the policies of this facility, which include a medical and physical examination
  • Must be able to relate to and work with ill, disabled, elderly, emotionally upset, and at time hostile people in the facility
  • Must be able to push, pull, move and / or lift a minimum of 25 pounds to a minimum height of 5 feet and be able to push, pull, move and / or carry such weight a minimum distance of 50 feet
  • May be necessary to assist in the evacuation of residents during emergency situations

WORKING CONDITIONS:

  • Works in office areas as well as areas throughout the facility
  • Moves intermittently during working hours
  • Is subject to frequent interruptions
  • Is involved with residents, personnel, visitors, government agencies / personnel, etc., under all conditions and circumstances
  • May be exposed to heat/cold temperatures in kitchen/storage areas
  • May be subject to sudden temperature changes when entering refrigerator/freezer
  • Communications with the medical staff, nursing personnel, and other department directors
  • Works beyond normal duty hours, on weekends and holidays when necessary.
  • Is subject to call back during emergency conditions (severe weather, evacuation, post-disaster, etc.)
  • Is subject to injury from falls, burns from equipment, odors, etc., throughout the workday, as well as reactions from dust, disinfectants, tobacco smoke, and other air contaminates
  • Is subject to exposure to infectious waste, diseases, conditions, etc., including TB and the AIDS and Hepatitis B viruses
  • May be subject to the handling of and exposure to hazardous chemicals
