The RN MDS Coordinator has overall responsibility to ensure complete, accurate and timely completion of the RAI process (including assessment and care plans) from admission to discharge, and maintain resident records adhering to state and federal regulations. The MDS Coordinator must be a role model for excellence in nursing and customer service and must be and must be able to motivate team members to meet a high standard.

PERFORMANCE REQUIREMENT / DUTIES AND RESPONSIBILITIES
Coordination of resident assessment, as well as care planning to provide the highest of quality care
Oversees the assessment process
Prepares assessment schedule and notifies all disciplines of said schedule
Ensures assessments and care plans are done accurately and in a timely manner
Prepare, file and track electronic paperwork submissions including required Medicare documentation
Works in collaboration with other staff members and departments
Monitoring charting and documentation for completeness & accuracy as necessary to ensure standards are met; review with CNA, licensed staff, therapy, recreation social service & dietary when necessary
Education of new staff on MDS process, MDS & care plan schedule and care planning process
Participating in State Survey
Determine clinical eligibility for Medicare A
Stay up to date with current guidelines and regulations
Care plan and assessment
Prints and distributes Quality Indicators to be reviewed & utilized for areas of improvement
Works with MMQ nurse to ensure complete and accurate assessments
Provides QM reports to DON for use in the QA process
Ensures that significant changes identified through the MDS process are communicated to the MMQ nurse, and other disciplines
Works collaboratively with other members of the interdisciplinary care team to provide holistic care
Notify unit coordinator, nursing supervisor or DON when CAN/Licensed Staff concerns are identified
Serve as a team leader to the care plan team

Meetings:
PPS-daily 8:15a-9a; review all Medicare residents, new events, new admissions- Brings sign in sheet
Care Plan- Tuesdays 1p-3p and Wednesdays 9a-11a- Brings charts, CPs and sign in sheet, update CPs prior to meeting
Risk- Thursdays 9a-10:30a
Falls- Every other Tuesday
General guidance for managing the MDS Process
Monday:
 Open all observations for today’s ARD
o Quarterly Norton plus
o Quarterly bowel and bladder assessment
o Quarterly pain assessment
o Quarterly fall risk assessment
 Interview for BIMS, pain, vision, hearing, etc. if able
 Pull care plans for this week’s meeting (new signature sheets if needed)
Tuesday
 Copy ADL flow sheets, positioning sheets, weekly skin sheets, behavior sheets & other data for MDSs (including Med A if needed)
 Interview staff for feedback if needed
 Review and update all care plans prior to care plan meeting at 1p
 Bring MDSs requiring signatures for the team to sign
 Care plan notes
Wednesday
 Finish data gathering & observation
 Check for completeness of MDS from other disciplines
 Update next week’s care plan
Thursday
 Complete MDSs and submit
 Open all MDS assessments for next week
Friday
 Print Facility Activity Report for all events and new and new orders for the previous week and check care plans for any needed update
 Chart audit follow-up
 Start the interview process for upcoming Monday quarterly assessments (BIMS, pain…)
Everything
 Complete a PPS tracker for all new Med A admissions and update for current Med A resident
 Enter all opened MDS assessments onto MDS log
 Email IDT when new MDS assessments onto MDS added to the schedule or if resident triggers for a significant change
 Notify members of the IDT when a CAA triggers for them
 Update care plans daily based on new or closed events, new orders, or other changes identified as PPS, Falls, care planning, and risk

EDUCATION REQUIREMENTS:
  • AANAC Certification within 6 months from start of position
  • Master’s degree in related field
  • Massachusetts RN License

EXPERIENCE:
  • Prior experience in MDS coordination
  • BLS certification
  • Long term care experience
