36 hrs/week. Tues/Thurs/Fri with 2 weekend days per month. 9am-9:30pm. The position will be cover hospitals and field, primarily Sarasota area but may need to travel throughout the 4 county coverage areas.

Primary tasks, duties, and responsibilities:
1. Promotes and practices the Hospice mission and values and follows policies and procedures of Tidewell Hospice.
2. Demonstrates knowledge of and adheres to the Florida Nurse Practice Act, Standards of Practice of Hospice Nursing as well as legal, regulatory and CHAP requirements.
3. Educates patient, family and caregivers regarding disease process and decline, palliative interventions, care giving, dying process, ethical concerns and safety practices.
4. Provides initial assessment of the impact of the terminal diagnosis on the patient’s physical, functional, psychosocial and environmental needs and activities of daily living.
5. Applies specific criteria to hospice care to establish appropriate levels of care and patient’s eligibility for admission and develops the initial plan of care and interventions to optimize the quality of life for patients and families.
6. Reviews with the resource and/or attending physician the care plan, all physician orders, and other records to ensure initial patient/family needs are met.
7. Accurately enters all admission data/orders/care plan into the electronic medical record (EMR) in a point-of-service environment within a 2 to 4 hour time frame. Secures proper signatures on all admission documents.
8. Demonstrates understanding of Interdisciplinary Group (IDG) concept, cooperating and working with all team members to ensure continuity of care.
9. Recognizes and maintains professional boundaries in relationships with patient/family/caregiver.
10. Successfully navigates within all patient care settings and complies with facility specific protocols.
11. Collaborates with the Access Center to exchange pertinent information and receive assignments.
12. Communicates timely and effectively with colleagues and utilizes voicemail or e-mail system where appropriate to all Tidewell team members in order for patient care to begin.
13. Ability to work within complex family relationships and dynamics.
14. Display respectful adherence to Tidewell’s community partner facilities, staff, rules and regulations.

Educational/Professional:
1. Must possess a current and unrestricted Florida Licensure as RN. BSN or MSN preferred.
2. Minimum one year experience in an acute-care Hospital / In-patient environment providing direct patient care; previous experience as a Hospice RN preferred.
3. CHPN certification is strongly encouraged within three years of employment.
4. An equivalent combination of professional experience and education may be considered as meeting the educational/professional requirements.
