In search of an experienced Resident Care Manager (RCM) for our skilled nursing facility. Must be a licensed nurse.


  • Will manage a unit with a max capacity of 42 residents
  • RN preferred, LPN with management experience will be considered
  • Long Term Care/SNF experience strongly preferred
  • Monday-Friday. May be asked to work weekends periodically.

Description of duties:
1. Monitors the quality of resident care on all the units to prioritize clinical support which is needed for the most clinically complex residents.
2. Coordinates clinical activities to provide optimum patient care on day and evening shifts.
3. Assists with evaluation of performance, counseling, and disciplinary action for the weekend staff
4. Assists with establishing standards of nursing care and ensuring quality of care provided meets standards.
5. Monitors safety practices of staff, safety conditions on the unit, and assures staff awareness and compliance with regulations and facility expectations.
6. Provides continuous feedback and information to the DNS regarding status and incidents occurring on the unit.
7. Coordinates and assures that appropriate investigation, documentation and reporting of incidents and accidents is consistently completed.
8. Is clinically competent, able to act as a program resource and is physically able to assist nursing staff with their responsibilities, or assist with emergencies as necessary.
9. Is familiar with facility and community infection control standards of practice, and monitors to assure compliance on assigned unit.
10. Consistently able to role model compliance with facility work habit requirements, i.e., attendance, punctuality, etc.
11. Demonstrates the ability to respond to incidents of conflict between residents, staff families etc, and effectively guide the resolution process
12. Monitors environment and care practices of the nursing personnel to ensure compliance with facility policy, and state and federal guidelines.
13. Develops strategies and processes that assure good interdisciplinary communication.
14. Participates in the collection and follow-up of customer satisfaction feedback.
15. Works collaboratively with all Dept. Managers to plan and deliver services to the unit.
16. Works with physicians and consultants to assure that residents are provided with appropriate care, monitoring and recommendations.
17. Monitors documentation and assures that it continues to meet facility, regulatory agency, and payer requirements.
18. Continuously remains familiar with resident status on the unit, and actively guides care if necessary.
19. Covers open LN shifts as needed

Physical/Sensory Requirements:

  • Must be physically capable of using four senses for assessment of resident status: touch, smell, hearing, seeing.
  • Must be physically capable of lifting 50 pounds occasionally and 20 pounds frequently.
  • Must be physically capable of pushing 50 pounds frequently.
  • Must be physically capable of pull 10 pounds frequently.
  • Must be physically capable of sitting for 30 minute intervals frequently.
