RN - UAS

General Purpose/Scope:

The UAS RN is a Registered Nurse who has expertise in UAS-NY Community Health Assessments. The UAS RN will be expected to assist in the enrollment process and complete comprehensive assessments on clients. The UAS RN will also be expected to comply with MLTC and UAS department policies. This role is a field position which requires the UAS RN to cover the Nassau and Suffolk County areas.

Responsibilities:

  • Conducts face to face or virtual assessment of members and potential enrollees utilizing the UAS Assessment Tool to determine functional status, medical, behavioral, psychological, and community resource needs.
  • Facilitates the assessment of clients as per MLTC policies.
  • Assesses potential enrollees/members’ community health needs and status.
  • Collaborates with Management team as needed to assist in the development of the Client’s Person-Centered Service Plan
  • Clarifies MLTC benefits, policies, and procedures for potential enrollees, current members, providers, and community-based agencies.
  • Adheres to documentation policies and procedures.
  • Utilizes UAS analyzer for improvement of assessment and documentation skills.
  • Informs potential enrollees of services Ideal Home Services Inc. able to provide.
  • Ability to perform other duties as assigned.

Qualifications:

  • Bachelor’s degree in Nursing, Health, Human Services preferred.
  • Current/active unrestricted NY State Licensed Registered Nurse.
  • Minimum of one year of experience in clinical practice.
  • Minimum of one year experience working with UAS assessment tool.
  • Minimum of one year experience working in MLTC, homecare, and or programs serving the frail and elderly (i.e., mobility, communication cognitive, or other impairments).
  • UAS Certification through New York State.

Skills/Abilities:

  • Proficient in Microsoft Office (Word, Outlook).
  • Excellent assessment and documentation skills.
  • Bilingual (verbal-Spanish) language ability preferred.
  • Strong communication skills, both written and verbal.
  • Knowledge of MLTC.
  • Ability to set priorities and multi-task.
  • Attention to detail with strong organizational and planning skills.
  • Proven ability to adhere to strict deadlines.
  • Must be able to make visits in two boroughs and/or counties.
  • Must have a valid driver’s license and vehicle for community travel.

Preferred Qualifications:

  • Certification in Care Manager – Accredited Case Manager (ACM) from American Case Management Association (ACMA) or Certified Case Manager (CCM) from Commission for Case Manager Certification (CCMC).
  • UAS Certification through New York State.
  • Experience in managed care and/or care management.
