JOB SUMMARY: Travel between Hiawatha Valley residential housing facilities in Winona to provide general physical and mental health care to clients who reside onsite.


JOB RESPONSIBILITIES AND ESSENTIAL FUNCTIONS:

  • Assist clients with medication management and administration in onsite medication rooms.
  • Conduct general health assessments and provide dietary or other physical health advice as necessary and appropriate.
  • Participate in groups with residents and other staff to educate others regarding healthy lifestyles.
  • Follow-up with residents on medical appointments and support collaboration of provider services as appropriate.
  • Establish collaborative, constructive working relationships with community service providers, residential employees and clients.
  • Provide consumer education on the basic principles of physical and mental health, as well as interpersonal and social skills, as assigned.
  • Attend appropriate conferences and in-service training sessions as necessary to meet licensure and program requirements.
  • Participate in clinical supervision as directed.
  • Transport clients as appropriate and directed by Program Director to support their mental and physical health needs.

PHYSICAL REQUIREMENTS FOR POSITION: Must be able to walk, sit, stand, crouch, twist, bend, reach overhead, maneuver stairs and stretch in a manner conducive to the execution of daily activities. Must be able to move around the office on a daily basis. While performing the duties of this job, the employee must communicate with others and exchange information. Employee regularly operates a computer and other office equipment on a daily basis. Occasional bending and lifting of office materials up to 30 lbs.


JOB QUALIFICATIONS AND REQUIREMENTS:

  • Registered Nurse or LPN
  • Meet the requirements of a Mental Health Practitioner. For purposes of this subdivision, a practitioner is qualified through relevant coursework if the practitioner completes at least 30 semester hours or 45 quarter hours in behavioral sciences or related fields or has obtained bachelor or master’s degree and: A. has at least 2,000 hours of supervised experience B. OR has 4,000 hours of supervised experience (without coursework) in the delivery of services to adults or children with

(i) mental illness, substance use disorder, or emotional disturbance; or (ii) traumatic brain injury or developmental disabilities and completes training on mental illness, recovery from mental illness, mental health de-escalation techniques, co-occurring mental illness and substance abuse, and psychotropic medications and side effects;

(ii) is fluent in the non-English language of the ethnic group to which at least 50 percent of the practitioner's clients belong, completes 40 hours of training in the delivery of services to adults with mental illness or children with emotional disturbance, and receives clinical supervision from a mental health professional at least once a week until the requirement of 2,000 hours of supervised experience is met; or

(iii) is working in a day treatment program under section 245.4712, subdivision 2

  • Must be at least 21 years of age.
  • Able to work with clients with severe and/or persistent mental illness, previous experience providing services to mentally ill clients preferred.
  • Exceptional organization and communication skills (oral and written). Decisive and accurate decision-making ability critical.
  • Team player, with strong ability to work with peers, clinical and administrative supervisor, community partners, and other clinical staff.
  • Possess a high level of human relation skills and the ability to work with and react appropriately to a variety of people in difficult or challenging situations. Ability to calmly and repetitively model appropriate behavior in difficult or challenging situations.
  • Must be able to maintain confidentiality.
  • Must possess a valid driver’s license and a willingness to travel as needed to client or organization locations throughout SE MN.

Have skills and knowledge acquired through at least one of the following:

  • a course of study in a health or human services related field leading to a bachelor of arts, bachelor of science, or associate’s degree;
  • one year of experience with the target population served;
  • experience as a certified peer specialist according to Minnesota Statutes, section 256B.0615; or
  • meet the requirements for unlicensed personnel under Minnesota Statutes, sections 144A.43 to 144A.483.
