Job Summary and Qualifications

As a case manager, you must focus on using strategies that are fiscally responsible, meaning they know how and where to get the necessary resources to deliver excellent care at a cost that is affordable. You will work together with patients and their families, but also with other physicians and partners such as the pharmaceutical and medical technology industry, as well as with insurance companies

About this unit:

  • Support the 25 Bed Facility
  • Swing Bed Level Offered, Skilled Care and Rehab Focus
  • Average of 12 to 15 Patients Per Day; with Winter months towards 25
  • Strong Team Oriented Unit; opportunity to support patients at the most critical time in a fast paced environment

Responsibilities:

  • The Continuum of Care Nurse/Case Manager position encompasses multiple responsibilities including utilization review, social services, coordination of Swing Bed activities, discharge planning, and performance improvement activities.
  • Performs reviews of inpatient medical records to ascertain that treatment is rendered in a cost effective manner while maintaining high quality care.
  • Assists patients &/or families with the social, emotional and financial concerns relating to health care; assessing the patients &/or problems & coordinates options available to meet these items.
  • Acts as a liaison to improve patient relations & improve customer service.
  • Assists with the hospital-wide employee health program.
  • Follows Joint Commission, CMS, and OSHA regulations.
  • Assists in preventative health programs such as employee vaccination programs, completes employee physicals and drug screens and assists in employee worker’s compensation cases.

What qualifications you will need:
  • Basic Cardiac Life Support must be obtained within 30 days of employment start date
  • Current State of Missouri RN licensure required. Licensed as R.N. by the Missouri State Board of Nursing or current and valid license from a compact state.

This role requires you to be fully vaccinated for COVID-19 based on local, state and /or federal law or regulations (unless a medical or religious exemption is approved).
