Mount Evans Home Health Care & Hospice is looking for full-time Lead Registered Nurse to be a vital member of our interdisciplinary Home Health Care and Hospice teams based in Evergreen, Colorado (located in the foothills only 20 miles from downtown Denver).

Ideal candidates are mountain-area residents with a reliable vehicle and all-season mountain driving experience.

About the position

This position will:

  • Support the Manager of Nursing Services and provide direction and leadership to staff and act as a resource to promote nursing in the community.
  • Oversee the LPN program and assist with staffing/supervising the LPNs.
  • Orient new team members including shadowing, EMR, and skills check list.
  • Attend daily huddles and provide support for Intake to staff nursing referrals.
  • Assist in weekend staffing.
  • Utilize reports to assist clinicians with documentation velocity.
  • Participate in QA and QAPI programs
  • Provide compassionate Home Health and Hospice nursing care in the home, in nursing facilities, and in assisted living facilities.
  • Assess and respond to the needs of patients and families in varied settings.
  • Communicate effectively with patients, families, and other members of our care team.
  • Travel to homes throughout the foothills of Clear Creek, Gilpin, Jefferson, and Park counties, primarily serving Evergreen and the 285 Corridor.
  • Work part-time, fleible hours, Monday – Friday.
  • Have a regular on-call requirement.

This position is based in our offices in Evergreen and also includes travel throughout our four-county service area.

This is an exempt position with starting pay in the $35.00 - $46.00 per hour range, depending on experience. Benefits package available includes medical, dental, paid time off, and more.

About you

You must (required):

  • Maintain an active Nursing license in the State of Colorado.
  • Have a minimum of two (2) years’ experience.
  • Be able to demonstrate your ability to provide effective and compassionate nursing care.
  • Be a self-managed team member with strong communication, organization, and time management skills.
  • Provide documentation of full COVID-19 vaccination or documentation requesting a reasonable exemption for medical or religious reasons.
  • Have a valid driver license, good driving record, proof of insurance, and reliable vehicle.
  • Have proof of legal right to work in the United States.

Ideally, you will (preferred):

  • Have experience in home health care, hospice, or palliative care.
  • Have a passion for end-of-life care.
