Key Responsibilities:
- Assessment of home health patient’s physical, psycho social, and environmental needs.
- Assessment of prior and current level of functioning.
- Setting appropriate goals.
- Making appropriate discharge recommendations in accordance with standards of practice.
- Demonstrate commitment, professional growth, and competency.
Minimum Qualifications:
Education:
- Graduate of an Accredited School of Nursing.
Licenses:
- Current Texas State License as an RN
- Valid Texas Driver’s License.
- Proof of current CPR accreditation.
Experience:
- Experience in the Home Care setting preferred.
Communication:
- Proficiency in English & Spanish preferred.

Physical Ability:
- Must be able to move and/or regularly lift up to 50 pounds.
Job Type: Full-time
Pay: $50.00 - $100.00 per hour
