Physician - Primary Care - Value Based Care - Nursing Homes

Our client is seeking a Primary Care Physician to travel to long term care/skilled nursing facilities in the Tucson, AZ area.

Schedule:

• Full Time: Monday – Friday, 8-4:30 PM (with flexibility)

Compensation: $190,000 - $240,000/yr base salary, dependent on experience, plus a 22.5% performance bonus

Responsibilities:

• Provide comprehensive primary care to 10-14 patients daily
• Prescribe and administer appropriate medications
• Educate patients and families on wellness, prevention, and early detection
• Provide referrals based on examination and patient needs
• Maintain patient electronic medical record data
• Collaborate with an interdisciplinary team including Advanced Practice Providers, Registered Nurses, Dietitians, Social Workers, etc.
• Assist with performance improvement work to provide care that reduces unnecessary hospitalization

Qualifications:

• Doctor of Medicine (MD or DO) and licensure to practice medicine in the State of Arizona
• Board certified in Internal Medicine, Family Practice or related discipline
• DEA license
• 1+ year of experience as a Physician, preferably in Primary Care
• Value based care experience, preferred

Job Type: Full-time

Pay: $230,000.00 - $295,000.00 per year
