POSITION SUMMARY: The Population Health Specialist will be responsible for collecting and reviewing patient population data and analyzing it to provide risk stratification. They will serve as a leader of population health based teams to ensure metrics are being addressed for the patient population. The Population Health Specialist will serve as the liaison to health plans and healthcare organizations participating in the care of our patients. Duties include:

• Operates systems such as CareSouth Carolina's EMR, health plan portals and population health analytics tools. Uses data to risk stratify patients and allocate appropriate resources to patients based on risk. Prepares and submits reports and other relevant work products timely and accurately.
• Works collaboratively with all CareSouth Carolina, Inc. staff to ensure the needs of the patient population are being met. Serves as a liaison for nursing and providers to assist with delivering care. Collaborates with providers, nursing, outreach, family support services, billing and coding to provide exceptional care to patients.
• Interacts with patients and employees in a professional and respectful manner consistent with the mission and values of CareSouth Carolina. Collaborate with other staff to achieve improved patient care and outcomes.
• Assists with the development and implementation of population health goals and strategies as required. Actively participates in meetings associated with population health.

SKILLS/EDUCATION REQUIREMENTS:

• A high school diploma or GED equivalent required. Two year degree preferred
• Proficient with electronic medical records and have the ability to provide detailed patient charting
• Medical office experience strongly preferred
• Strong interpersonal & communication skills & the ability to work effectively with a wide range of constituencies in a diverse community
• Proficient in Microsoft Office applications - Word, Excel, PowerPoint
• Self-directed, flexible, and comfortable working independently
• Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists
• Ability to interpret a variety of instructions furnished in written, oral, and diagram or schedule form
• A valid driver's license required

Hours: 8:30 am – 5:00 pm Monday – Friday

Location: Dillon, SC

Proof of full vaccination for COVID-19 must be presented prior to beginning work with CareSouth Carolina. Full vaccination is receipt of both doses of the Moderna or Pfizer vaccine or one dose of the Johnson & Johnson vaccine.
