Under the direct supervision of the Director of Client Services, the Peer Support Specialist will aid the YHDP Case Manager and youth/young adults to move toward self-advocacy in the attainment of their own, client driven life goals. This will be done using the four tasks of Intentional Peer Support: building connection, helping each other understand how we’ve come to know what we know, redefining help as a co-learning and growing process, and helping each other move toward what is wanted rather than away from what is not wanted. This position is responsible to work off site at a variety of partnering agencies within Baltimore City and assist with the completion of intakes and case management services.

PRIMARY DUTIES AND RESPONSIBILITIES

· Work directly with the YHDP Case Manager in providing direct care case management services to (25) youth and young adults annually.

· Provide peer support, information and referrals regarding housing, legal and financial resources to transitional aged youth ages 18-24.

· Provide referrals for community based services including job readiness trainings, individual or group therapeutic services.

· Participate in ongoing Outreach activities within the case management program as assigned.

· Participate in the planning and implementation of group activities as assigned.

· Maintain collaborative working relationships all partnering agencies and other that SCS refers to.

· Participate in scheduled supervisory sessions with YHDP Case Manager and the Director of Client Services

· Other duties as assigned.

GENERAL DUTIES

· Active participation in team meetings to assist with ensuring the clients receive appropriate and timely services and to provide input to enhance service delivery for program participants.

· Keep notes on victims updated to the day in the confidential electronic files system used by the agency.

· Maintain statistics regarding service delivery and other services provided to clients as necessary.

· Maintain awareness of community resources.

· Manage tasks assigned.

SUPERVISORY RESPONSIBILITIES

This position has no supervisory responsibilities

EDUCATION and/or EXPERIENCE

· High School or GED Required

· Be a young adult (18-25 years old) with experience handling complex emotional and behavioral needs.

· Have life experience which allows you to possess a diverse understanding of the struggles youth with complex emotional needs endure.

· Have successfully navigated child serving systems (mental health, child welfare, probation, education).

· Ability to become a credentialed Youth Support Partner.

· Knowledge and understanding of the ten guiding principles of wraparound and their implementation in service planning.

· Demonstrated excellent verbal and written communication skills.

· Proficiency with computer applications including Microsoft Word, Excel and Power Point.

· Current Driver’s License

· Safe Driving Record

LANGUAGE SKILLS

Ability to write routine reports and correspondence. Ability to speak effectively before groups of clients, collaborative agencies or employees of the Agency.

MATHEMATICAL SKILLS

Basic math skills of addition, subtraction, multiplication and division are required.

CERTIFICATES, LICENSES, REGISTRATIONS

· Current Maryland Driving License

· Safe Driving Record

· The peer support specialist must be able to drive to other officers operated by SCS to attend meetings and trainings that are required by this position.

· The Peer Support Specialist will work in the community at various partnering locations in office based setting. The Peer Support Specialist will need to travel to meetings, hotels, etc. to meet with clients and attend meetings. This must be done in a timely and efficient manner.

Job Type: Full-time

Pay: $16.80 - $18.00 per hour
