JOB SUMMARY:

Reporting to the Quality Improvement Coordinator, the Patient Outreach Lead supports the QI Department’s planning, implementation, and evaluation of quality improvement projects. Project activities include helping to develop and test scripts for outreach strategies, conduct patient outreach using high tech and traditional methods, support patients and remove barriers to care, evaluate progress using the Model for Improvement (PDSA), communicate project challenges and opportunities, and communicate project outcomes across stakeholder groups. The Patient Outreach Lead will also support staff training and documentation of procedures as directed.

ESSENTIAL DUTIES

• Implementing outreach campaigns using phone, postal, email, and digital messaging services;
• Using scripts to inform patients of needed preventative care services
• Translating scripts from English to Spanish in a culturally competent manner
• Providing patients with helpful information to support patient follow-through
• Asking pertinent questions to support patient needs
• Recording the patient’s personal information accurately into the EMR
• Meeting daily/weekly performance goals
• Scheduling patient visits;
• Documenting and communicating process outcomes;

• Coordinating with interdisciplinary teams and across departments to close care gaps

QUALIFICATIONS/EXPERIENCE:

1. Prior experience conducting patient outreach required

2. Prior experience with using a digital messaging service (preferred)

3. Basic skills using Excel, Teams, Outlook, and SharePOINT required.

4. Experience using population health platforms and EHR required

5. Excellent communication skills

6. Must be proficient in Spanish speaking and writing

7. Patient and able to handle customer questions, complaints, and rejection

8. Highly motivated, team player, self-directed

9. Able to execute tasks in a quickly changing environment

10. Strong calling to service for vulnerable populations

11. High school diploma preferred
