JOB SUMMARY

Assists patients of all age groups with activities of daily living, ensures the safety of the patient through frequent rounding and positioning, and provides basic patient care to include vital signs, meal delivery, and assisting with patient ambulation. Performs EKGs, starts IVs, if needed, performs lab draws through either venipuncture or skin puncture, collects other specimens, and checks blood glucose levels under the direction of the Registered Nurse. Watches telemetry monitors and answers phones for med/surg and ICU units. Notified nurse if abnormal heart rhythm is noted. Ensures the patient’s safety, comfort, and routine care is provided. Immediately supervised by Registered Nurse and Charge Nurse on shift. Reports to MedSurg/ICU Director. Non-Exempt Position.

ESSENTIAL FUNCTIONS

Performs Nurse Aide, Phlebotomy, and EKG duties within the associated scope of practice.

Provides basic patient care such as bed baths and modified bed baths, showers and feeding.

Performs all safety checks during bedside shift report and reviews all documentation with oncoming shift.

Assists in admission, transfer and discharge of patient, including setting up room.

Notifies appropriate licensed personnel when patient complains of pain, abnormal vital signs, or signs of skin breakdown (decubitus).

Records and documents urine input & output, weights, and vital signs accurately into the electronic health record.

Answers call lights promptly, and performs hourly rounding to ensure patient has water, has assistance in going to the restroom, if needed, updates on food, and is at a comfortable temperature (room temperature, sheets/blankets, etc.).

Ensures bedrails are up and all safety precautions are followed, including bed alarms.

Provides explanation to patient and family prior to administering care to patient; answers questions within scope of practice, and finds assistance when additional explanation is needed.

Follows proper procedures to report damaged product/equipment/instrumentation (lock-out/tag out). Enters work orders for equipment and other identified needs.

Medical and surgical asepsis protocols are carried out during treatments and special procedures.

Performs Accuchecks on patients to check blood glucose levels and documents accordingly.

Performs other duties as assigned, within the PCA’s scope of practice, such as Range of Motion, Ambulation and safe patient transfers using the transfer belt and patient lifts.

Assist in transporting patients within the unit and to and from other units as needed.

Properly uses selected equipment and monitors patient environment.

Provides explanation to patient and family prior to administering care to patient; answers questions within scope of practice, and finds assistance when additional explanation is needed.

Monitors and discontinues foley catheters and calls for assistance when needed.

PHLEB

Able to start and discontinue IVs within the scope of practice.

Performs specialty blood draws on patients as needed, including blood gases, blood culture collections, etc.

Performs lab draws on patients using 2 patient identifiers before a venipuncture or skin puncture is performed.

Labels and records all test tubes, logs procedures, and documents time, date, and location accurately and timely within the Electronic Health Record.

Separates and disposes all sharps in appropriate waste container.

Maintains inventory of supplies within the unit. Coordinates order with laboratory and unit.

Requires minimal supervision in performance of job duties.

Prioritizes orders and patient care and assists nurses when needed.

Reviews patient requisitions and records to verify tests are correct with what was drawn, and no tests have been missed or incorrectly drawn or ordered.

Ensures phlebotomy tray/carts are fully stocked and surrounding areas are maintained, clean and clutter free, including the phlebotomy drawing room, outpatient waiting area, and processing area.

Able to draw PKUs and prepare documentation for lab to document and mail to Dept of State Health Services.

Responsible for outpatient faxes (i.e. verifies all results have been faxed to appropriate location)

EKG

Attaches electrodes of Holter monitor to patient to record data over extended period of time.

Perform EKGs according to procedure and monitors EKG for abnormal patterns.

Manages and operates equipment safely and correctly. Reports all malfunctions.

Notifies respiratory therapist or registered nurse immediately if patient complains of pain.

MONITOR TECH

Answers the telephone and nurse call system timely and professionally and transfers calls accordingly.

Communicates information about dysrhythmias to the charge nurse and primary care nurse.

Scans medication orders to the pharmacy department.

Runs and measures rhythm strips at the specified times.

Maintains restraint log daily.

Schedules follow up appointments and assembles discharge packets.

Keeps patient assignment boards up to date.

Prepares medical records for new admissions and discharged patients.

Logs all patients upon admission, transfer, discharge or expiration.

Communicates information accurately and promptly to charge nurse, co-workers and physicians.

Orders supplies and nourishments and puts them away promptly.

Monitors rejected orders on a daily basis.

Maintains current downtime forms.

Enters work orders as needed for the unit.

POSITION REQUIREMENTS

Minimum Education

High School Graduate or GED required if no higher education is obtained.

Certified Nurses’ Assistant (CNA), or Clinical Certified Medical Assistant (CCMA) required. One year of experience preferred.

Completion of Phlebotomy program required. Phlebotomy Technician certification (AMT, ASPT, or equivalent) required in 6 months.

Current BLS for healthcare professionals required at time of hire.

Monitor tech certificate or UMH training and passage of testing in reading EKG strips within 1 month of hire.

Required Skills

Must possess superior ability to communicate effectively in English, both verbally and in writing. Additional languages preferred.

Excellent interpersonal skills and customer service.

Must possess high degree of organization and be able to multi-task efficiently and effectively.

Intermediate computer knowledge required, including proficiency in Microsoft Office and Outlook.

Ability to use standard office equipment (copier, fax, etc.).

FUNCTIONAL DEMANDS

Functional Job Requirements:

Job requirements include, but are not limited to: sitting, standing, walking, kneeling, bending, squatting and reaching. Requires good finger dexterity, repetitive motions with hands and fingers. Requires ability to have good vision and hearing. Pushing and pulling equipment may be necessary.

Lifting Requirements

Job requires heavy lifting on an occasional basis.
