Rehab Medicine at Dartmouth Hitchcock Medical Center in Lebanon, NH is looking for a per diem physical therapist to provide evidence based physical therapy in our outpatient Orthopedic clinic. You will be a part of a team of specialists that work collaboratively with the Ortho and Primary Care providers at Dartmouth Health. This position offers an opportunity for mentorship and the ability to work alongside experienced clinicians.

Responsibilities
  • Examines, evaluates, diagnoses, determines prognosis and provides intervention. Reports findings to other health care team members. Provides plan of care recommendations, exercise programs, and family teaching.
  • Provides patient care 1:1 with a team of therapists including Physical Therapy Assistants
  • Participates in and supports in-service education, student programs, volunteer/observation programs, etc.
  • Completes electronic documentation and billing within Epic EMR.
  • Coordinates the scheduling of patients and meetings.
  • Determines and implements annual plan for professional development.
  • Performs other duties as required or assigned.

Qualifications
  • Bachelor's degree if graduate prior to 2002; minimum of Master's degree if graduating in 2002 or later.
